Psychiatric Comorbidity in Alcohol Dependence

Zusammenfassung

Anliegen Psychiatrische Komorbidität kann den Beginn und den Verlauf einer Alkoholabhängigkeit ungünstig beeinflussen. Methode Wir untersuchten 384 alkoholabhängige entgiftete Patienten mittels strukturiertem diagnostischen Interview. Ergebnisse Häufigste Störungen: Nikotinabhängigkeit (NA; 42 %), antisoziale Persönlichkeit (ASP; 23 %), Dysthymie, Phobie (je 10 %). Alkoholabhängige mit komorbider NA oder ASP waren jünger zu Beginn der Abhängigkeit und konsumierten signifikant höhere Alkoholmengen. Schlussfolgerungen NA und ASP erfordern bei der qualifizierten Entgiftungsbehandlung besondere Beachtung.

Abstract

Objective It is thought that psychiatric comorbidity could influence onset and course of alcohol dependence. Methods We investigated 384 alcohol-dependent subjects, admitted for in-patient detoxification, by using a structured diagnostic interview to determine prevalence of axis-I- and axis-II disorders. Results 42 % of all subjects met the criteria of tobacco dependence (TD). 10 % of alcoholics showed dysthymia or phobia. There were surprisingly low rates of depression (6 %), post traumatic stress disorder (2,3 %) or anxiety disorder (2,1 %). 22,7 % met the criteria of antisocial personality disorder (ASP). Predominantly, alcoholics comorbid to TD or ASP had earlier onset of alcoholism with higher intake of alcohol. Conclusions Nearly every second alcohol-addicted patient undergoing an inpatient detoxification has a tobacco dependence and nearly every fourth has an ASP, most of latter met the criteria of Cloninger's type-2-alcoholism (83 %). Therapeutic attention to the symptoms of personality disorder and TD may reduce the propensity to relapse.
