Combination of a Supratentorial Venous Anomaly and Infratentorial Developmental Venous Anomalies Mimicking AV-Malformation: A Case Report

Abstract

Developmental venous anomalies (DVAs) are considered to be congenital anatomical variants of venous drainage. In most cases unspecific symptoms lead to their diagnosis. We describe a 35-year-old patient presenting with headache and dizziness. MRI was suspicious for arteriovenous malformation and the dorsal horn of the right lateral ventricle was enlarged. Digital subtraction angiography showed complex venous malformations with abnormal supratentorial venous drainage and bilateral cerebellar DVAs with associated varix nodes. This is a unique case of complex supra- and infratentorial abnormal venous drainage. After considering the complexity of these venous anomalies and the fact that the patient was asymptomatic there was no reasonable indication for treatment.

Zusammenfassung

Venöse Anomalien werden als kongenitale anatomische Variationen der venösen Drainage betrachtet, häufig wird ihre Diagnose aufgrund unspezifischer Symptome in der Bildgebung gestellt. Wir beschreiben einen 35-jährigen Patienten, der durch Kopfschmerzen und Schwindel auffällig war. Kernspintomographisch zeigte sich neben einer Ausziehung des Hinterhornes des rechten Seitenventrikels der Verdacht auf eine AV-Malformation. Die digitale Subtraktionsangiographie zeigte komplexe venöse Anomalien mit einer atypischen supratentoriellen Drainage sowie bilaterale zerebelläre DVAs mit jeweils assoziierten Varixknoten. Dies ist ein bislang einzigartiger Fall einer kombinierten supra- und infratentoriellen venösen Anomalie. Bei dem asymptomatischen Patienten ergab sich angesichts der Komplexität der Malformation keine Behandlungsindikation.
