Aussagewert von Funktionsdaten für Indikation und Kontraindikation in der Lungenchirurgie

Zusammenfassung

Vor einer geplanten Lungenresektion muß grundsätzlich eine Lungenfunktionsuntersuchung durchgeführt werden. Sie soll Aussagen über die Resektionstoleranz und über das Komplikationsrisiko ermöglichen. Die Indikation zu funktionsverbessernden Thoraxoperationen, wie Dekortikation oder Bullektomie, hängt geradezu vom Ergebnis der Funktionsanalyse ab. Für die präoperative Untersuchung wird ein Stufenprogramm vorgeschlagen, das mit der Messung einfacher spirographischer Werte beginnt. Je nach dem Ausfall dieser Werte sind methodisch schwierigere Untersuchungen, wie Ergometrie, Blutgasanalyse, Pulmonalisdruckmessung und schließlich Bronchospirometrie erforderlich. Die Beurteilung von Bronchialkarzinom-Patienten enthält eine besondere Problematik, an deren Beispiel Möglichkeiten und Grenzen der Funktionsdiagnostik demonstriert werden.

Summary

An examination of pulmonary function must be carried out before performing a pulmonary resection. It should reveal information concerning tolerance of the resection and complication risk. The indication to resections intended to improve function such as decortication or bullectomy depend directly on the results of the functional analysis. A stepwise programme for the preoperative examination is proposed that begins with the measurement of simple spirographic values. Depending on the results obtained, more difficult examinations such as ergometry, blood gas analysis, measurement of pressure of the pulmonalis and finally bronchospirometry are then necessary. The evaluation of patients with bronchial carcinoma which is particularly problematic is used to demonstate the possibilities and limits of functional diagnosis.
