Temporäre Anwendung der Ösophagusendothese vor Operation eines Karzinomrezidivs nach Kardiaresektion

Zusammenfassung

Bericht über eine 45jährige Patientin, bei der es 4œ Jahre nach Kardiaresektion wegen Karzinom zu einem Rezidivtumor kam. In 1. Sitzung temporäre Einlage einer Ösophagusendothese per laparatomiam, in 2. Sitzung linksseitig thorakale Nachresektion von Speiseröhre und Magen, Ösophagusverschluß, atypische Gastrostomie, in 3. Sitzung abdomino-thorakale Speiseröhrenrekonstruktion mit Colon ascendens und transversum und in 4. Sitzung operativer Verschluß der Magenfistel.

Summary

The case of a 45 – year old patient who suffered a recurrence of a tumour 4œ years after cardiectomy for carcinoma is reported. The following four successive operations were performed: temporary insertion of an oesophageal endoprosthesis by laparotomy; right lateral thoracic resection of oesophagus and stomach, closure of the stomach and atypical gastrostomy; abdominal – thoracic reconstruction of the oesophagus with the ascending and transverse colon; closure of the gastric fistula.

Résumé

Présentation du cas d'une malade de 45 ans atteinte d'une récidive du cancer pour lequel elle a subi une résection du cardia quatre ans et demi auparavant. Dans un premier temps une laparatomie permit de poser une plastie oesophagienne; dans un second temps il fut procédé dans la région thoracique droite à une résection complémentaire de l'oesophage et de l'estomac, à une suture de l'oesophage et à une gastrostomie atypique; dans le troisième temps eut lieu une plastie oesophagienne abdominothoracique à l'aide du colon ascendant et transverse et dans le 4e temps à une fermeture de la fistule gastrique.
