Zusammenfassung
Aneurysmata der Koronararterien werden selten klinisch diagnostiziert, da sie lange bestehen können, ohne Symptome hervorzurufen. Bei einer 52 Jahre alt gewordenen Patientin, die unter den klinischen Zeichen eines akuten Cor pulmonale bei Lungenembolie verstorben war, ergab die Obduktion fünf große Aneurysmata beider Koronararterien. Als Ursache kommen vorwiegend die schwere Arteriosklerose, möglicherweise auch kongenitale Faktoren einer Elastica-Hypoplasie an den Aufteilungsstellen in Betracht. Die Differentialdiagnose hat vor allem verkalkende Mediastinalprozesse und verkalkte Vorhofthromben zu berücksichtigen.
Summary
Aneurysms of the coronary arteries are rarely diagnosed clinically because they can exist for a long time without giving rise to signs or symptoms. The case is reported of a 52-year-old woman who died with the signs and symptoms of acute cor pulmonale, but post-mortem examination revealed five large aneurysms in the coronary arteries. Severe arteriosclerosis is the predominant cause of such aneurysms, but congenital hypoplasia of the elastic membrane at the arterial branchings is another possible cause. Calcific mediastinal disease and calcified atrial thrombi have to be considered in the differential diagnosis.

Aneurismas múltiples calcificados de ambas arterias coronarias
Los aneurismas de las arterias coronarias se diagnostican clínicamente con rareza, ya que pueden existir mucho tiempo sin dar síntomas. En una enferma de 52 años de edad que había fallecido bajo los signos clínicos de un cor pulmonale agudo por embolia pulmonar mostró la autopsia cinco aneurismas grandes de ambas coronarias. Como causas se han de considerar sobre todo las arteriosclerosis graves y posiblemente también los factores congénitos de una hipoplasia de la túnica elástica en los lugares de distribución. El diagnóstico diferencial tiene que tener en cuenta en primer lugar los procesos mediastínicos calcificantes y los trombos calcificados de las aurículas.
