Zusammenfassung
Der Chlorpromazin-Ikterus verläuft unter dem klinischen Bild eines intrahepatischen Verschlusses. Es wird über 14 eigene Beobachtungen berichtet, bei denen einmalig oder mehrmals Leberpunktionen durchgeführt wurden. Histologisch ist das Bild sehr monoton und durch Parenchymzellveränderungen und Gallenthromben ohne entzündliche Infiltrate gekennzeichnet. Pathogenetisch wird der Ikterus auf Grund der Histologie und der Bilirubinanalyse auf einen primären Leberzellschaden zurückgeführt. Die Gallenthromben sind nicht Ursache, sondern Folge dieser Schädigung. Offen bleibt, ob das Medikament direkt toxisch oder über einen Sensibilisierungsmechanismus die Leberzelle schädigt.
Summary
Jaundice caused by chlorpromazine clinically takes the course of an intrahepatic obstruction. 14 personal cases are reported in which one or repeated liver punctures were obtained. The histological picture was uniform, characterized by changes in the parenchymal cells and biliary thromboses, but without inflammatory infiltrates. The primary cause of the jaundice is thought to be liver damage: biliary thromboses are its consequence. But the question remains undecided whether the drug has a direct toxic effect or sensitizes the hepatic cell to the harmful action of some other substance.

Ictericia por cloropromazina, una forma de hepatosis colostásica
La ictericia por cloropromazina cursa con las características clínicas de una obstrucción intrahepática. Los autores refieren 14 observaciones propias en las cuales se practico biopsia hepática en una o más ocasiones. El cuadro histológico es extraordinariamente monótono y se caracteriza por alteraciones de las células parenquimatosas y trombos biliares, sin infiltrados inflamatorios. Desde el punto de vista patogénico se considera, en vista de los hallazgos histológicos y el resultado del análisis de la bilirrubina, que la ictericia se debe a una alteración primaria de las células hepáticas. Los trombos biliares no son la causa sino la consecuencia de esta lesión. No se sabe con seguridad todavía si el medicamento lesiona las células hepáticas por una acción tóxica directa o a través de un mecanismo de sensibilización.
