ORTHOSTATIC TEST IN ESTIMATION OF EFFICACY OF MATERNAL AND FETAL HEMODYNAMICS IN PREGNANT WOMEN WITH FETOPLACENTAL INSUFFICIENCY

Purpose: The aim of our study was to analyze if it is possible to detect the latent disturbances of blood flow in pregnant women with fetoplacental insufficiency by orthostatic test.

Methods and Materials: Doppler examinations (S/D ratio) of the maternal uterine and fetal umbilical and middle cerebral arteries before and after orthostatic test (15s and 15 minutes) and also ultrasound fetometry were performed in 152 women with physiological pregnancy and in 156– with pregnancy complicated by fetoplacental insufficiency between 14 and 40 weeks of gestation.

Results: It was found, that at physiological pregnancy, the changes of S/D ratio in fetal arteries on orthostatic test did not exceed 11%. At fetoplacental insufficiency the amplitude of S/D changes exceeded 11%. In such situation complications of pregnancy (intrauterine growth restriction –41.6% of cases, chronic fetal hypoxia –62.3%) were registered 2.5 times more frequently, than in cases of uncomplicated pregnancy.

Conclusions: At early stages of fetoplacental insufficiency decrease in stability of hemodynamic subsystem of mother-placenta-fetus complex can be found. Orthostatic test can be effectively utilized for assessment of the latent disturbances of blood flow in pregnant women.
