Stumpfes Bauchtrauma: Milzlappung als Differentialdiagnose einer Milzruptur

Zusammenfassung

In einer retrospektiven Studie wurden die Daten von 232 Patienten mit stumpfem Bauchtrauma in der Zeit vom 1. 1. 1984 bis zum 31.12. 1988 analysiert. In diesem Zeitraum fielen sonographisch 4 Patienten mit einer glatt begrenzten, harmonischen Konturunterbrechung der Milz und freier Flüssigkeit in der Milzloge auf, bei denen differentialdiagnostisch eine Milzruptur auszuschließen war. Es handelte sich dabei jeweils um eine Milzlappung als anatomische Variante der Milz, die in 2 Fällen zu einer ungerechtfertigten Probelaparotomie unter der Verdachtsdiagnose einer Milzruptur führte. Bei der sonographischen Notfalldiagnostik ist somit an die Möglichkeit einer Milzlappung als Differentialdiagnose einer Milzruptur zu denken.

Abstract

In a retrospective study, we analyzed the data of 232 patients with blunt abdominal trauma between January 1, 1984, and December 31, 1988. In emergency ultrasound, we detected 4 patients with a smooth disrup-üon of the splenic outline combined with free peritoneal fluid around the spleen. Bccause of the blunt abdominal trauma, a rupture of the spieen had to be excluded. In two of these 4 cases, we decided for laparotomy. Intraopera-tively, we found a splenic lobulation and no splenic injury in both cases. We conclude that an emergency sono-graphic examination of the spieen, it should be kept in mind that the anatomical Variation of a splenic lobulation might mimic a rupture of the spieen.
