MR-Angiographie der Beckenvenen und der Vena cava inferior bei 0,5 Tesla

Zusammenfassung
Bei 15 Patienten mit Beckenvenenthrombosen oder retroperitonealen Tumoren wurde die venöse Beckenstrombahn und/oder die V. cava inferior zusätzlich zur Phlebo- bzw. Cavographie mit einer MR-Angiographie untersucht. Die MR-Angiographie erfolgte bei 0,5 Tesla in Inflow-Technik (TR/TE 30/8 ms, Flipwinkel 70°). Aus den 2-D-Angiogrammen wurden koronale 3-D-Projektionsangiogramme erstellt. Bei überwiegend guter Bildqualität ließen sich die Venen bis zur V. iliaca interna in allen Fällen darstellen, darunter dagegen nur unzuverlässig. Der Vergleich der Methoden miteinander bezüglich der therapeutisch relevanten Aussage zeigte, daß in knapp 2/3 aller Untersuchungen (8/13) MR-Angiographie und Phlebo- bzw. Cavographie in der diagnostischen Aussage gleichwertig waren. Beim Vorliegen eines Nierenzellkarzinoms ist die Differenzierung zwischen Pelottierung und Infiltration der V. cava anhand der MR-Angiographie unsicher.
Summary
The pelvic veins and /or the inferior vena cava were examined in 15 patients with pelvic vein thrombosis or retroperitoneal tumours, using phlebography or cavography and MR angiography. The latter was carried out using 0.5 Tesla with an inflow technique (TR/TE 30/8 ms, flip angle 70°). 3-D projection angiograms were obtained from the 2-D angiograms in the coronary plane. Image quality was generally satisfactory and the veins up to the internal iliacs could be demonstrated in all cases; below this, the examination was unreliable. Comparison of the methods with reference to information of therapeutic relevance has shown that in two-thirds of all examinations (8/13) MR angiography and phlebography were of equal diagnostic value. In the presence of a renal cell carcinoma, MR angiography was unable to distinguish between thrombosis and infiltration of the vena cava.
