Intoxikation mit Gallopamil: Verlauf und Therapie

Zusammenfassung

Eine 32jährige Patientin wurde etwa 2 Stunden nach Einnahme von 7 g des Calciumantagonisten Gallopamil somnolent in der Klinik aufgenommen. Bei der Aufnahme betrug der systolische Blutdruck etwa 40 mm Hg bei bestehendem AV-Block III. Grades. Zur Therapie der Hypotonie erwiesen sich Orciprenalin, Dopamin und Dobutamin sowie Gaben von Calcium als unwirksam. Nach anfangs massiven und über 26 Stunden fortgeführten Gaben von Adrenalin und Noradrenalin gelang es, den Blutdruck zu normalisieren und mit passagerem Schrittmacher die Rhythmusstörung im Laufe von 24 Stunden zu beseitigen. Die Gallopamil-Plasmakonzentration lag etwa 4 Stunden nach Einnahme mit 8,4 µg/ml um etwa den Faktor 100 über dem therapeutischen Bereich.

Abstract

About two hours after swallowing 7 g of Gallopamil, a calcium antagonist, with suicidal intent a 32-year-old woman was admitted to hospital in a somnolent state. At that time the systolic blood pressure was about 40 mm Hg, in the presence of a 3° A-V block. Orciprenaline, dopamine and dobutamine, as well as calcium, proved ineffective. Initially massive doses of adrenaline and noradrenaline, and their continued administration over the subsequent 26 hours, succeeded in normalizing the blood pressure and after transitory pacing, terminated, the arrhythmia in the course of 24 hours. About four hours after its intake the plasma gallopamil concentration was 8.4 µg/ml, about 100 times the therapeutic range.
