Bilateral mucoepidermoid carcinoma of the parotid gland – case report

    Introduction:

    The mucoepidermoid carcinoma (MEC) is the most common malignant tumour of the salivary glands. In most cases (> 80%) MEC arise from the parotid gland. Women are more frequently affected than men and the mean age at diagnosis is around 47 years. Clinically in most cases a palpable tumour is noticed. A bilateral appearance is very rare.

    Methods:

    case report, literature research.

    Results:

    A 41-year-old woman who had suffered from a non-Hodgkin-lymphoma presented with a non-painful tumour of her left parotid gland. After a subtotal parotidectomy a cystic mucoepidermoid carcinoma of the left parotid gland pT1cN0cM0 was diagnosed (low grade, MAML2 translocation, close margins). Six months later two further tumours were seen in the right parotid gland. After a ductal carcinoma in situ of the left mamma had been diagnosed an increased size of the tumours of the right parotid gland led to a subtotal parotidectomy on the right. Again, a mucoepidermoid carcinoma pT1cN0cM0 with cystic parts (low grade, MAML2 translocation, close margins) was diagnosed.

    Discussion and conclusion:

    In several studies a translocation of the MAML2 gene was found in more than 80% of the MEC cases and was declared as a sensitive and specific marker of the MEC. This translocation was often associated with a low-grade tumour and better overall survival rate. Bilateral malignant tumours of the parotid gland are very rare but need to be considered as a differential diagnosis.
