Evaluation of 100 Drug induced sleep endoscopies (DISE) to find the best individual treatment of OSA

    Introduction:

    CPAP therapy is known as the gold standard forpatients with obstructive sleep apnea (OSA). About 1/3 of the patients who are treated with CPAP don't use their therapy regularly or seldom. Therefore it is necessary to provide other options for treatment. Next to polysomnography we use drug induced sleep endoscopy (DISE) to evaluate for the best individual treatment of OSA.

    Materials & Methods:

    In 100 patients (73 men and 27 women) at the age of 22 – 87 years we used DISE to evaluate the best treatment option for OSA. Our patients had an average BMI of 25,68 and an average AHI of 36,48/h. DISE is an upper airway technique which we carry out in the operating theatre. All of the 100 DISE were evaluated with the VOTE-classification.

    Results:

    We accomplished 100 DISE in 77% of patients who were evaluated for CPAP therapy and in 23% to find out alternative ways of treatment. About 73% had an average age of at least 50 years. Only CPAP-Therapy was indicated in 23% of the proceeded DISE. We evaluated the hypoglossal nerve stimulation as the most common alternative way of treatment. Patients who were treated with CPAP-therapy show a high compliance (74%) in their follow up visits.

    Discussion:

    DISE is a good method to find out the best and individual treatment options for OSA. There are different types of noninvasive (CPAP-therapy, lower jaw brace) or invasive (hypoglossal nerve stimulation, uvulopalatopharyngoplasty, reduction of the tongue base tonsil) procedures. Because of the individual recommendation and procedure of OSA therapy in our clinic we have a good compliance in OSA therapy.
