Presentation of the HaptiVisT ear surgery training system - current status and evaluation

    Introduction Operations of the temporal bone represent a special challenge for the ENT surgeon. The aim of the BMBF-funded project was to develop a realistic and resource-saving training system for ear operations in the shape of a "serious game".

    Methods The current prototype of the HaptiVisT system functions as an otosurgical training system with visual feedback via a glasses-free 3D monitor and haptic feedback via a haptic arm simulating the drill. The division into three surgical levels (antrotomy, mastoidectomy and posterior tympanotomy) ensures a variety of training options. A weighted scoring system enables the measurability of the training success. A prospective evaluation was performed with a standardized questionnaire (scaling from 1=very good to 5=very bad) in cooperation between Fraunhofer IIS Erlangen and the ENT University Hospital Leipzig.

    Results A total of n=12 subjects with an average practical experience in ENT medicine of n=4.75 years participated in the evaluation. The usefulness for the medical further training was evaluated with 1.33 on average. In particular, the training system was considered helpful for learning anatomy (average grade 1.58) and hand-eye coordination (average grade 1.67). The graphic and acoustic aspects were rated as good and the haptics as good to moderate.

    Conclusion The ear surgery training system "HaptiVisT" offers the possibility of realistic and resource-saving training of ear operations. An integration into the clinical daily routine and in particular into the medical further training (to the ENT-specialist) seems reasonable.
