Long-term Results after Surgery of Cholesteatoma with Canal Wall Down (CWD) Mastoidectomy and Bony Obliteration of the Mastoid

    Background To assess long-term results and present experience with a bone patè for obliterating large open mastoid cavities compared to surgical outcome of the bony mastoid and epitympanic obliteration technique with canal wall down reconstruction in adults (Vercruysse, 2016) with primary surgery of a cholesteatoma or revision surgery.

    Methods The data of 559 patients treated in our department for surgery of cholesteatoma or revision surgery from 01/2014 to 12/2018. Eighty seven patients (age from 18 - 91 yr) were included.The revision rates of both patient groups were analyzed.

    Results Patients were reexamined after a minimum follow-up period of 12 months after obliteration of the open mastoid cavity with bone patè. Recurrent Cholesteatoma occured in 6,9 5. Difficult differentiation between recurrent cholesteatoma and granulation tissue in follow-up examinations and in CT scan was observed. Other main outcome measures are pure-tone average, postoperative status of the ear and long-term safety issues.

    Conclusion Data showed a lower recurrent cholesteatoma rate in the comparison group to our data (6,9 zu 2 %). The Canal Wall Reconstruction (CWR) with bony obliteration of the mastoid and epitympanon seems a positive effective option for treatment of large open cavities.
