The analysis of perioperative risk factors on postoperative complications and death after hip fracture surgery in senile patients

    Objectives Objective: To identify the risk factors which may lead to postoperative complications or death after hip fracture surgery in senile patients and discuss their inherent relationships.

    Methods The consecutive 265 patients with hip fracture surgery, aged 65 years or older, were retrospectively evaluated in this study. The patients’ gender, age, type of fracture, comorbidities, surgical timing, anesthesia method, surgical type and perioperative blood transfusion were compared in a multivariate logistic regression analysis to identify independent predictors of one-year morbidity and mortality.

    Results and Conclusion One-year mortality was 10.57% and morbidity was 58.49%. Compared with the patients without preoperative comorbidities, the patients suffered from three or more comorbidities showed a significantly increase trend in incidence of postoperative complications (P=0.000, OR=4.793). Meanwhile, the results indicated that both the patients with timing of surgery more than 72 hours after injury (P=0.001, OR=3.836) and the patients with more than three comorbidities (P=0.011, OR=7.752) showed a dramatic rise in mortality with statistically differences. Other factors did not show visible effect on the postoperative complications and survival situation of the patients.

    Conclusion: Among the risk factors in this study, a relationship only can be found between the preoperative comorbidities and the postoperative complications, while both the preoperative comorbidities and the surgical timing are risk factors which can lead to the death in senile patients. Base on above, the senile patients with hip fracture should be treated with surgery as soon as possible to improve their prognosis by reducing the morbidity and mortality related to postoperative complications.
