Ruptured intradiploic dermoid cyst into an extra dural hematoma in a traumatic brain injury patient

Abstract
Traumatic rupture of occipital dermoid cyst into the extra dural hematoma (EDH) is an extremely rare event leading to management difficulty. We report an interesting case of ruptured intradiploic dermoid cyst into EDH in a 62 year-old male who presented with history of moderate head injury due to assault along with rupture of a pre-existing occipital dermoid cyst. On surgery, a ruptured cystic mass was encountered which was in communication with the extradural space being filled with blood clot and cheesy white pultaceous sebaceous material. Traumatic brain injury in our case caused rupture of the dermoid leading to dissemination of cyst material into the extradural space as well as out of the scalp surface confusing with leaking brain matter. Judicious and timely surgical intervention is warranted in such cases for optimal clinical outcome.
