Nekrolytisches migratorisches Erythem – Ein Fallbericht

Zusammenfassung

Es wird über einen 58-jährigen Mann mit therapieresistenten makulopapulösen Effloreszenzen an den Unterschenkeln sowie multiplen, krustig belegten Papeln im Gesichts- und Rückenbereich berichtet. Auffällig waren zudem eine B-Symptomatik und eine chronische Anämie ohne bekannte Malignomerkrankung. Diese Hinweise ließen eine Paraneoplasie vermuten und schließlich die Diagnose eines nekrolytisch migratorischen Erythems (NME) im Rahmen eines Glukagonom-Syndroms stellen. Der Fall zeigt die Herausforderung für die Diagnosestellung eines NME bei wiederholt unspezifischer Dermatopathologie und initial negativ verlaufender Tumorsuche.

Abstract

We report a case of 58-year-old man with therapy-resistant maculopapular lesions on the lower legs and multiple crusty papules on the face and back. B-symptoms and chronic anemia without known malignancy were also present. These hints suggested a paraneoplasia and led finally to the diagnosis of a necrolytic migratory erythema (NME) as part of a glucagonoma syndrome. The case shows the challenge of diagnosing NME in the context of unspecific dermatopathology and an initially negative tumor screening.
