Aseptische Nekrosen des Kniegelenkes
Die Osteonekrose – auch als Knocheninfarkt bezeichnet – ist eine schwerwiegende Erkrankung, die zu einer fortgeschrittenen Arthrose führen kann. Die Behandlung fokussiert sich darauf, den Progress der Erkrankung und damit die Entwicklung einer Arthrose zu vermeiden, wobei spontane und postarthroskopische Osteonekrosen initial konservativ und sekundäre Osteonekrosen am besten chirurgisch behandelt werden.

Knochennekrosen – auch als Knocheninfarkt bezeichnet – beschreiben den Gewebeuntergang von Knochengewebe, durch den ein Defekt entsteht.

Man unterscheidet am Kniegelenk
-
spontane Osteonekrosen (SPONK) (vaskulär und traumatisch verursacht),
-
sekundäre Osteonekrosen (vor allem durch Kortikosteroide, Alkoholmissbrauch, Sichelzellanämie, SLE, Morbus Gaucher, Hämophilie u. a. bedingt) und
-
postarthroskopische Osteonekrosen (PAONK) (nach Arthroskopie).

Die SPONK wird neuerdings auch als subchondrale Insuffizienzfraktur bzw. SIFK (Subchondral Insufficiency Fracture of the Knee) mit sekundärer Nekrose betrachtet.

Die klinischen Befunde sind bei SPONK nicht immer eindeutig, bei sekundären Osteonekrosen und PAONK geben vor allem die Anamnese im Einklang mit den klinischen Beschwerden den Hinweis.

Die bildgebende Diagnostik umfasst zunächst eine Röntgenuntersuchung, bei anhaltender Symptomatik ist das MRT indiziert.

Die sogenannte Fensterperiode (Window Period) beschreibt das Zeitfenster zwischen Beginn der Symptome und positivem MRT-Befund. Die Bedeutung für den klinischen Verlauf ist noch unklar.

Bei SPONK und PAONK erfolgt die Therapie zunächst konservativ (Teilbelastung, NSAID).

Bei sekundären Osteonekrosen mit klinischen Beschwerden bringt die konservative Behandlung in der Regel keine Linderung.

Chirurgische gelenkerhaltende Maßnahmen (Entlastungsbohrung, Umstellungsosteotomie) sind bei Patienten im Präkollapsstadium indiziert. Im Endstadium erfolgt der Gelenkersatz (Hemischlitten, Totalendoprothese).

Kleinere und mittlere Läsionen haben eine gute Prognose, größere Defekte eine schlechtere.
