Zusammenfassung
Die vorliegende Kasuistik beschreibt der Therapieverlauf einer Patientin mit high
grade serösem Ovarialkarzinom, FIGO IIIB und BRCA1-Mutation und einer
Defizienz der homologen Rekombination. Die Standardtherapie nach aktueller
Leitlinie sieht hier 6 Zyklen Paclitaxel und Carboplatin in Kombination mit
Bevacizumab vor. Neu ist die mittlerweile etablierte Erhaltungstherapie mit
Bevacizumab in Kombination mit dem PARP-Inhibitor Olaparib. Diese
Kombinationstherapie ist indiziert bei den in diesem Fall vorliegenden
genetischen Veränderungen. Das supportive
komplementärmedizinische Nebenwirkungsmanagement fokussiert sich auf die
bei dieser Therapie wesentliche Nebenwirkung, die Chemotherapie-induzierte
Neuropathie (CIPN). Hier kommt mit der Hilotherapy® ein neues
apparatives Kühlungssystem zum Einsatz, dessen Effektivität in
mehreren Studien evaluiert wurde. Ergänzt wird dieser Ansatz durch die
systemische Behandlung mit Endocannabinoiden, antiinflammatorischen
Fettsäuren und alpha-Liponsäure.
Abstract
This case report describes the therapeutic course of a patient with high grade
serous ovarian cancer, FIGO IIIB and BRCA1 mutation and a deficiency of
homologous recombination. The standard therapy according to current guidelines
provides 6 cycles of paclitaxel and carboplatin in combination with bevacizumab.
New is the now established maintenance therapy with bevacizumab in combination
with the PARP inhibitor olaparib. This combination therapy is indicated for the
genetic alterations present in this case. Supportive complementary medicine
focuses on the management of the major side effect associated with this therapy,
i. e. chemotherapy-induced neuropathy (CIPN). The so called
Hilotherapy® is a new cooling system applied during
chemotherapy. Its effectiveness has been evaluated in several studies. This
approach is complemented by systemic treatment with endocannabinoids,
anti-inflammatory fatty acids and alpha-lipoic acid.
