Hintergrund

Die orthoplastische Chirurgie spielt eine zentrale Rolle in der Versorgung von Arbeits- und Wegeunfällen unter dem Dach der gesetzlichen Unfallversicherung. Dieser Beitrag analysiert aktuelle Standards in Akutversorgung, septischen Verläufen und Rehabilitation.

Patienten/Material und Methoden

Ausgewertet wurden Leitlinien der Deutschen Gesetzlichen Unfallversicherung (DGUV), Zertifizierungsanforderungen für Traumazentren sowie retrospektive Fallserien aus zugelassenen Kliniken (2015–2024).

Ergebnisse

Moderne Behandlungspfade integrieren plastisch-rekonstruktive Techniken in 78% der Schwerstverletztenfälle (SAV). Septische Komplikationen nach Polytrauma zeigen eine Inzidenz von 9–12%, wobei einstufige Wechseloperationen bei 82% der Protheseninfektionen erfolgreich sind. Die interdisziplinäre Rehabilitation verkürzt die Arbeitsunfähigkeitsdauer um durchschnittlich 23%.

Schlussfolgerung

Die Vernetzung von Akutkliniken, septischer Chirurgie und Rehabilitationszentren optimiert Outcomes. Standardisierte Qualitätsindikatoren und telemedizinische Nachsorgekonzepte bedürften weiterer Forschung und Entwicklung.

Abstract

Background

Orthoplastic surgery is pivotal in managing occupational accidents under German statutory accident insurance. This review examines current standards in acute care, septic complications, and rehabilitation.

Patients/Material and Methods

Guidelines from the German Social Accident Insurance (Deutsche Gesetzliche Unfallversicherung, DGUV), certification requirements for trauma centres, and retrospective case series from accredited hospitals (2015–2024) were analysed.

Results

Modern treatment pathways incorporate plastic-reconstructive techniques in 78% of severely injured patients (SAV). Septic complications after polytrauma occur in 9–12% of cases, with single-stage revision surgeries achieving success in 82% of prosthetic infections. Interdisciplinary rehabilitation reduces average sick leave duration by 23%.

Conclusion

Networking between acute care facilities, septic surgery units, and rehabilitation centres improves outcomes. Standardised quality metrics and telemedicine follow-up concepts require further research.
