Zusammenfassung
Um eine Mangeldurchblutung des Gehirns während Operationen an der Arteria carotis zu vermeiden, sind protektive Maßnahmen notwendig: Vollnarkose mit Konstanterhaltung des Blutdrucks, normale CO2 -Spannung, Verwendung eines intraluminalen Shunt und Heparinisierung. Operative Behandlung „asymptomatischer“ Patienten muß in Erwägung gezogen werden. Einen akuten Verschluß einer Arteria carotis interna soll man nur operativ behandeln, wenn die Operation direkt im Anschluß an das akute Ereignis durchgeführt werden kann. Ein chronischer Totalverschluß der Arteria carotis interna kann mit Endarteriektomie behandelt werden, wenn der Siphon durchgängig und in gutem Zustand ist.
Summary
Optimal protection of the brain during carotid endarterectomy is of great importance and is guaranteed by general anaesthesia with maintenance of normal blood-pressure and a normal C02-level, routine use of internal shunts and heparinization. Operation of “asymptomatic” patients has to be seriously considered. Patients with acute occlusions of the carotid artery should not have surgery as emergency procedures, unless the operation can be done immediately after the onset of symptoms. In chronic occlusion of the internal carotid artery endarterectomy may be considered if the shiphon is patent and in good condition.
