Zusammenfassung

Restaurative Eingriffe im Bereich des linken Ausflußtraktes bei Säuglingen, Kindern und Jugendlichen stellen den Operateur insofern vor ein schwerwiegendes Problem, als er in Anbetracht des Alters dieser Patienten die abnormen anatomischen Gegebenheiten möglichst schonend verändern soll, um eine annäherd normale Hämodynamik zwischen linkem Ventrikel und Aorta wieder herzustellen. Nicht selten ist dabei eine Klappenschlußunfähigkeit wechselnden Ausmaßes der Preis, den man im Interesse des Patienten in Kauf nimmt, um einen prothetischen Aortenklappenersatz im Kindes- und Jugendalter zu umgehen. Etwa 10–15 % dieses Krankengutes kommen daher später für einen Aortenklappenersatz in Frage.

In 10 Jahren wurden unter 2000 Herz-Lungen-Maschinen-Operationen 255 Säuglinge, Jugendliche und Kinder an verschiedenen Formen von Aortenstenosen und Läsionen im Bereich des linken Ausflußtraktes operiert. Im einzelnen wurden dabei 222 Aortenstenosen unterschiedlicher Lokalisation und 20 Sinus von Valsalva Aneurysmen operiert, außerdem wurde in 13 Fällen ein prothetischer Aortenklappenersatz vorgenommen. Die Gesamtletalität betrug 9,3 %, wobei der höchste Anteil auf die mit einer begleitenden Fehlbildung kombinierten Formen fiel.

Im einzelnen gliedert sich das Krankengut folgendermaßen auf: Von 135 valvulären Aortenstenosen verstarben insgesamt 5,9 %; bei 10,3 % dieses Kollektivs waren zusätzliche Herzgefäßanomalien vorhanden. Als Folge der Kommissurotomie entstand in 8 % der Fälle eine Klappeninkompetenz. — 28 Patienten hatten eine supravalvuläre Aortenstenose; 25 % dieser Fälle gingen mit einer begleitenden Läsion einher. Die Gesamtletalität dieser Gruppe betrug 14,3 %. In 3,6 % verblieb eine Reststenose. Die supravalvuläre Aortenstenose wurde längs inzidiert und mittels einer Flickenplastik erweitert. — Bei 36 Patienten mit einem subvalvulären Diaphragma fand sich die höchste Quote (30,6 %) begleitender Fehlbildungen. Hier betrug die Letalität 8,3 %. Bei 5,5 % verblieb eine Restinsuffizienz. Das subvalvuläre Diaphragma wurde entweder transaortal oder durch einen begleitenden Ventrikelseptumdefekt ausgeschält. — Von 23 idiopathischen hypertrophischen Subaortenstenosen zeigten nur 13 % eine selbständige, begleitende Mißbildung. Die Gesamtletalität betrug bei diesem Kollektiv 21,7 % und war damit am höchsten. Allerdings verstarb von den acht zuletzt, nach der Kombinationsmethode von Bigelow und Cooley operierten Patienten, keiner mehr. In 4,4 % der Fälle verblieb eine Reststenose. — Als Haupttodesursache in der Gruppe der kongenitalen Aortenstenosen fand sich das Linksherzversagen. — Von den 13 Kindern und Jugendlichen, bei denen ein prothetischer Aortenklappenersatz vorgenommen wurde, verstarben vier in tabula, dreimal durch unbeherrschbare Blutung. In der überwiegenden Anzahl dieser Fälle wurde eine Starr-Edwards-Klappe eingesetzt, wegen primärer oder operativ verursachter Aortenklappeninsuffizienz. — Im Kollektiv der 20 korrigierten Sinus von Valsalva Aneurysmen war kein Todesfall zu verzeichnen. Der Ausgangsort der SVA war 13mal der rechte Sinus, 3mal der linke und 4mal der neutrale Sinus. In 9 Fällen waren Perforationen in die Herzhöhle aufgetreten, einmal iatrogen bedingt.

Summary

Restorative Operations in the region of the left effluent tract in infants, children and adolescents present a difficult problem to the surgeon because of the age of the patient. The abnormal anatomical situation should be changed with as much care as possible in order to obtain a haemodynamic situation between the left ventricle and aorta which approaches normal. A deficient valvular closure of varying degree is often the price which has to be paid in order to avoid a prosthetic replacement of the aortic valve in children and adolescents. About 10–15 % of these patients come into question for aortic valve replacement within 10 years.

There were 255 infants, children and adolescents with various forms of aortic stenosis and lesions in the region of the left effluent tract among a total of 2000 patients operated over a 10 year period with the heart-lung machine. These included 222 aortic stenoses of varying location and 20 aneurysms of the sinus of valsalva. A prosthetic replacement of the aortic valve was performed in 13 cases. The total morality was 9.3 %, most of the deaths occuring in patients with concomittant malformations.

The case material could be analysed as follows:

Out of 135 patients with valvular aortic stenosis, 5.9 % died. An additional malformation of the cardiac vessels was present in 10.3 % of this group. There was valvular incompetence as a result of commissurotomy in 8 % of the cases. 28 patients had a supravalvular aortic stenosis and 25 % had a accompanying lesion. The total mortality in this group was 14.3 %. There was a residual stenosis in 3.6%. The supravalvular aortic stenoses were incised longitudinally and broadened by means of a patch-plasty. The highest quotient (30.6%) of concomittant malformations was found in 36 patients with subvalvular diaphragm. The mortality was 8.3 %. There was residual insufficiency in 5.5 %. The extirpation of the subvalvular diaphragm was either trans-aortal or through an accompanying ventricle septum defect. Only 13 % of 23 idiopathic hypertrophic sub-aortic stenoses showed an independent concomittant malformation. The total mortality was highest in this group with 21.7 %. However, none of the last 8 patients operated by the combined method of Bigelow and Cooley died. There was a residual stenosis in 4.4 %. Left heart failure was the main cause of death in the group with congenital aortic stenosis. Of the 13 children and adolescents who underwent a prosthetic replacement of the aortic valve, 4 died on the operating table, 3 of them due to uncontrollable bleeding. A Starr-Edwards valve was inserted in the majority of these patients on account of a primary aortic valve insufficiency or one caused by operation. In the group of 20 corrected aneurusms of the sinus of valsalva, there were no deaths. The site of origin of the aneurysm was the right sinus in 13 patients, the left in 3 patients and the neutral sinus in 4 patients. In 9 cases there was perforation into the cardiac cavity, one of these being caused iatrogenically.

Des interventions de plastie au voisinage de la valvule aortique chez le nourrisson et chez l'enfant posent des problèmes graves à l'opérateur, car il doit opérer le plus parcimonieusement possible les anomalies anatomiques en raison de l'âge de l'opéré, afin de rétablir un hémodynamisme à peu près normal entre le ventricule gauche et l'aorte. Il en résulte fréquemment une insuffisance valvulaire plus ou moins importante, que l'on doit accepter pour éviter la mise en place d'une prothèse de la valvule aortique chez l'enfant. L'indication de la mise en place d'une prothèse aortique se pose ainsi ultérieurement dans le 10 à 15 % de ces enfants.

Au cours de 10 ans il a été procédé à des opérations de 255 nourrissons et enfants sur un ensemble de 2000 opérations avec l'appareil coeurpoumon. Il s'agissait de diverses formes de stenoses aortiques et de lésions au voisinage du départ de l'aorte. 222 sténoses aortiques on été opérées et 20 anévrismes du sinus de Valsalva; dans 13 cas il a été procédé à la mise en place d'une prothèse de la valvule aortique. La mortalité totale a été de 9,3 %, la plus élevée provient du groupe des sténoses aortiques combinées à d'autres malformations.

L'ensemble des cas se répartit de la façon suivante: la mortalité fut de 5,9% sur 135 sténoses aortiques; sur cet ensemble le 10,3 % présentait d'autres malformations cardiovasculaires. La commissurotomie fut accompagnée d'une insuffisance valvulaire dans le 8 % des cas. 28 malades avaient une sténose supravalvulaire; le 25 % de ces cas présentait une lésion accessoire. La mortalité totale de ce groupe fut de 14,3 %. Il y eut une sténose résiduelle dans le 3,6 % des cas. La sténose aortique supravalvulaire fut traitée par une incision longitudinal et élargie par une plastie — Sur l'ensemble des malades avec un diaphragme sous-valvulaire se rencontre la fréquence la plus élevée (30,6%) d'autres malformations conjointes. La mortalité fut de 8,3 %. Dans le 5,5 % des cas il restait une sténose résiduelle. Le diaphragme sous-valvulaire fut extirpé à travers l'aorte ou à travers une communication interventriculaire existante. — Sur l'ensemble des 23 sténoses idiopathiques hypertrophiques sousaortiques il n'y avait que 13 % d'autres malformations. La mortalité totale de ce groupe fut élevée, elle atteint 21,7%. Aucun ne mourut toutefois après l'utilisation de la technique de Bigelow et Cooley. Dans le 4,4 % de ces cas il y eut après l'intervention une sténose résiduelle — La cause principale de la mort dans le groupe des sténoses aortiques congénitales fut l'insuffisance du coeur gauche. — Sur les 14 enfants soumis à une intervention avec mise en place d'une prothèse de la valvule aortique 4 sont morts sur la table d'opération, trois en raison d'une hémoragie qui ne put être maîtrisée. Il a été procédé dans le plus grand nombre des cas à la mise en place d'une prothése de Starr-Edwards en raison d'une insuffisance primaire ou secondaire postopérative de la valvule aortique. — Sur l'ensemble des 20 cas de correction des anévrismes du sinus de Valsalva il n'y eut aucun décès. L'anévrisme provenait 13 fois du sinus droit, 3 fois du gauche et 4 fois du sinus neutre. Dans 9 cas il y eut perforation dans la cavité cardiaque, une fois d'origine iatrogène.
