Perforation von Schrittmacheraggregaten in den Bauchraum als seltene Komplikation der chirurgischen Elektrotherapie

Zusammenfassung

An zwei Beispielen wird die ungewöhnliche Perforation eines vor der hinteren Rektusscheide im linken Oberbauch implantierten Schrittmacheraggregats in den freien Bauchraum beschrieben. Trotz dieser mit 0,6 % seltenen Komplikationsmöglichkeit wird der Implantationstechnik in der beschriebenen Weise gegenüber der subkutanen Implantation der Vorzug gegeben. Subkutan implantierte Schrittmacheraggregate perforieren ungleich häufiger durch die vordere Bauchwand. Bei einer derartigen, stets mit Infektion kombinierten Komplikation sind in den meisten Fällen die Entfernung des gesamten Systems und Neuimplantation erforderlich. Da in den beiden beschriebenen Fällen keine Infektion eingetreten ist, genügte eine Neuimplantation des perforierten Aggregats bzw. ein Aggregatwechsel unter Belassung des Elektrodenteils.

Summary

Two examples of perforation of a pacemaker which had been implanted anterior to the posterior rectal sheath in the left upper abdominal quadrant into the peritoneal cavity are described. Because of this rare complication (0.6%), the implantation technique described here ispreferred to subcutaneous implantation. Pacemakers implanted subcutaneously perforate more frequently through the anterior abdominal wall. When this complication occurs, it is often combined with infection and removal of the entire system and reimplantation is necessary. As there was no infection in the two cases described, reimplantation of the perforated aggregate or change of the aggregate was performed while the electrodes were left in situ.
