Ungewöhnliche operative Rekanalisation der Trachea beim Zylindrom

Zusammenfassung

Es wird über die Rekanalisation der Trachea in 2 Fällen von Zylindrom berichtet, wobei in einem Fall die thorakale Trachea in ganzer Länge im Bereich der Hinterwand gespalten, im anderen Fall der tumortragende Lappen herausgeklappt worden ist. Die Tumorabtragung erfolgte in beiden Fällen mit der Diathermieschlinge. In Kenntnis der Yasargilschen Versuche über Trachealprothesen und trotz Bewunderung der Leistung von MATHEY U. Mitarb. wäre im eigenen Fall 2 sicherlich Teil- oder Total-resektion möglich, im Fall 1 aber ebenso sicher undiskutabel gewesen. Die Spaltung der Hinterwand ist im übrigen bei den seltenen Fällen der hochgradig stenosierenden Tracheopathia osteoplastica, wie auch bei einigen trachealen Amyloidosen zu erwägen, denn man konnte in Fall 1 die größeren derben Tumorknollen sehr gut mit einem kleinen Luer abknabbern.

Summary

Report on the recanalisation of the trachea in two cases of cylindroma. In one case the total length of the thoracic portion of the trachea had to be split at the dorsal aspect, in the second case the tumor bearing part was excised and folded out. Resection of the tumor was done with the electric knife in both cases. Despite knowledge of YASARGIL'S work on tracheal protheses and admiration for the achievements of MANTHEY et al., the resection of the trachea was considered impossible in case one, whereas partial or total resection could have been quite possible in case two. Splitting of the dorsal wall of the trachea is to be taken into consideration in those rare cases of a highly stenosing osteoplastic tracheopathy as well as in some cases of tracheal amyloidosis. In case one it was possible to remove the larger and very firm nodules of the tumor with a small bone-cutting forceps.

Résumé

Nous communiquons deux cas de recanalisation de la trachée pour cylindrômes. Dans le premier cas c'était la partie postérieure de la trachée qui était rongée sur toute sa longueur. Dans le deuxième cas le tissu tumoral s'extériorisait. L'enlèvement de la tumeur est effectué dans les deux cas au bistouri électrique. Ayant pris connaissance de l'essai de Yasargilsche sur la prothèse trachéale et grâce à l'admirable travail de MATHEY et de ses collaborateurs la tumeur pouvait être réséquée partiellement ou totalement dans le 2° cas. Ce fut également le cas d'une façon indiscutable dans le 1° cas. L'atteinte de la partie postérieure de la trachée est du reste dans de rares cas, à considérer comme la trachéopathie ostéoplastique la plus sténosante. Il y en est de même pour quelques cas d'amyloïdose trachéale. On pouvait cureter dans le 1° cas très facilement avec une curette cette masse tumorale très dure.
