Zusammenfassung
Eine 72jährige Diabetikerin erkrankte nach dreijähriger Tolbutamid-Therapie an einer
Panzytopenie. Nach Absetzen des Tolbutamids und Einhalten der bei Panzytopenie üblichen
Therapie bleibt die Anämie unverändert bestehen, während Leukopenie und Thrombozytopenie
ausgeglichen werden konnten. Wenn solche Panzytopenien unter Tolbutamid auch extrem
selten sind, ist mit aller Eindringlichkeit auf eine Gewichtsreduktion und strenge
Diätführung als erste therapeutische Schritte in der Diabetesbehandlung hinzuweisen.
Beim Einsatz von Sulfonylharnstoffen sind Blutbild- und Thrombozytenkontrollen nach
10 Tagen, alle drei Monate und später jährlich notwendig.

Pancytopenia developed in a 72-year-old woman after three years of tolbutamide treatment.
When tolbutamide was discontinued and the usual treatment of pancytopenia undertaken,
the anaemia persisted while the leukopenia and thrombocytopenia were reversed. Pancytopenia
during tolbutamide treatment is extremely rare. But the blood picture and platelet
count should be regularly checked in all patients receiving sulphonylurea drugs, ten
days after starting treatment, every three months for the first year and then once
yearly.
