Zusammenfassung
Die Bestimmung des Glomerulumfiltrats ist eine häufig indizierte Untersuchung zum
Erfassen einer bisher unbekannten Funktionseinschränkung der Nieren aus diagnostischen
Gründen, zur adäquaten Dosierung renal eliminierter, potentiell toxischer Medikamente
und zur Beobachtung des Verlaufs chronischer Nierenerkrankungen und nach Nierentransplantation.
Wie aus Untersuchungen an 85 Patienten hervorgeht, kann das Glomerulumfiltrat, gemessen
mit der Inulinclearance, mit hinreichender Zuverlässigkeit durch das Serumkreatinin
geschätzt werden. Der Fehler der Schätzung ist ungefähr gleich groß wie jener der
Kreatininclearance. Kreatinin wird nach einer neuen Modifikation von Bartels und Mitarbeitern
bestimmt, die sich in jedem Laboratorium einfach durchführen läßt und damit die Schätzung
des Glomerulumfiltrats einem weiteren Kreis von Ärzten als bisher ermöglicht. Die
Methode erweist sich jener mit dem Autoanalyzer Technicon II ebenbürtig.
Summary
Measurement of glomerular filtration rate is a commonly indicated investigation. It
is used to diagnose the degree of previously unknown limitation of renal function,
to determine the adequate dose of potentially toxic drugs excreted through the kidneys
and to observe the course of chronic renal disease and renal function after transplantation.
Investigations in 85 patients showed that the glomerular filtration rate as measured
by inulin clearance can be estimated with sufficient reliability from the serum creatinine.
The margins of error of the estimation are approximately the same as those of the
creatinine clearance. Creatinine is measured according to a new modification by Bartels
and co-workers which can be easily performed in every laboratory. The method has proved
itself to be as accurate as that of the autoanalyser Technicon II.

Valoración del filtrado glomerular por la creatinina sérica
La determinación del estado glomerular es un análisis indicado frecuentemente para
detectar una limitación renal funcional, hasta entonces desconocida, con fines diagnósticos,
para dosificar adecuadamente el medicamento de eliminación renal y potencialmente
tóxico y para observar la evolución de las afecciones renales crónicas y tras el transplante
renal. El filtrado glomerular, medido con el aclaramiento de la inulina, puede ser
valorado con suficiente validez por la creatinina sérica según se deduce de las exploraciones
en 85 pacientes. El error de valoración es aproximadamente igual al del aclaramiento
de la creatinina. La creatinina se determina según una nueva modificación de Bartels
y colaboradores, que puede ser llevada a cabo con facilidad en todo laboratorio, facilitando
así la valoración del filtrado glomerular a un mayor círculo de médicos que hasta
ahora. El método se mostró de la misma calidad que el auto analizador Technicon II.
