Zusammenfassung

Bei einer 67jährigen Patientin, bei der eine ausgeprägte Arteriosklerose bestand, wurde eine retrograde Aortendissektion erstmals angiographisch nachgewiesen; später bei der Obduktion wurde die Diagnose histologisch bestätigt. An der retrograden Dissektion sind wahrscheinlich mehrere Faktoren beteiligt gewesen: Die Intimalazeration befand sich genau oberhalb des Abganges des Truncus coeliacus, wobei die Wandtextur dem eindringenden Blut den Weg nach distal versperrt haben kann. Auch der niedrigere Druck im Thorax mit einer gewissen Sogwirkung auf das Blut in der Gefäßwand könnte an der atypischen Richtung der Dissektion mitgewirkt haben. Eine entscheidende Rolle bei der Entstehung der Initimaverletzung dürfte die Arteriosklerose gespielt haben.

Summary

Retrograde aortic dissection was demonstrated angiographically in a 67-year-old woman who had severe arteriosclerosis. The diagnosis was confirmed histologically at post-mortem examination. Several factors were probably responsible for retrograde dissection: the intimai tear was just above the origin of the coeliac axis, the structure of the wall probably prevented distal flow of the dissecting blood stream. The lower thoracic pressure with a certain suction effect on the blood in the vessel wall may also have contributed to the atypical direction of the dissection. Arteriosclerosis probably was the decisive factor in the causation of the intimal tear.

Aportación al problema de la disección retrógrada de la aorta

En una paciente de 67 años de edad afecta de una notoria arteriosclerosis se constató angiográficamente por primera vez una disección aórtica retrógrada: diagnóstico que fue confirmado histológicamente más tarde en la autopsia. En la disección retrógrada habían participado probablemente diversos factores: la laceración de la íntima se encontraba exactamente por encima de la salida del tronco celíaco, pudiendo haber cerrado el tejido parietal el paso en sentido distal a la entrada de la sangre. También la más baja presión en el tórax con un cierto efecto de succión sobre la sangre en la pared vascular podría haber colaborado en la dirección atípica de la disección. La arteriosclerosis podría jugar un papel decisivo en el desarrollo de la lesión de la íntima.
