Zusammenfassung

Es wird ein Überblick über die neurologische Symptomatologie des multiplen Myeloms gegeben. Ein umschriebenes kraniales Myelom kann zu einer Lähmung der betroffenen Hirnnerven führen, das Bild eines Hirntumors oder auch eines Tumors der Orbita hervorrufen. Ein Myelom des Rückenmarks kann die Symptome einer Mark- oder Wurzelkompression bieten. Pathogenese und Morphologie werden besprochen: die myelomatöse Neuropathie, die umstrittenen myelomatösen „Toxine”, vaskuläre Komplikationen und Änderungen der Zerebrospinalflüssigkeit. Das Myelom greift niemals auf das Nervengewebe selbst über, zentralnervöse Symptome sind also stets auf eine indirekte Wirkung, im allgemeinen einen Druck des Tumors von außen her, zurückzuführen. Es ist wichtig zu wissen, daß neurologische Ausfälle die ersten Symptome einer multiplen Myelose sein können, deren Generalisation sich erst später entwickelt. Nur die spinalen Symptome des Myeloms können vorübergehend durch eine Operation oder eine Bestrahlung zur Rückbildung gebracht werden, im übrigen ist im allgemeinen keine wirksame Behandlung möglich.

Summary

The neurological aspects of multiple myelomatosis have been surveyed and involvement of each part of the nervous system considered in turn. The local pressure of a cranial myeloma will produce a syndrome of multiple cranial nerve palsies or syndromes simulating those of cerebral or orbital tumour. A spinal myeloma may produce spinal cord or root compression. Each condition is described and myelomatous neuropathy, myelomatous “toxins”, vascular complications of the disease and, finally, the cerebrospinal fluid changes are dealt with. Unlike other malignant conditions and blood dyscrasias, myelomatous tissue never invades nervous tissue and its effects are therefore always indirect and usually due to pressure. The reason for this behaviour presents an interesting problem, presumably related to the biology of the myelomatous Tissue. It is of clinical importance to note that any of the neurological manifestations may be the presenting features of multiple myelomatosis, and the generalized disease develops later. Except for the spinal myelomas, where temporary relief may be possible by surgery and radiation therapy, treatment is at the moment almost invariably unsuccessful.

Síntomas neurológicos de la mielomatosis múltiple

Se da un resumen sobre la sintomatología neurológica del mieloma múltiple. Un mieloma craneal circunscrito puede producir una parálisis de los nervios encefálicos correspondientes, un cuadro de tumor cerebral o también el de un tumor de la órbita. Un mieloma de la médula espinal puede mostrar síntomas de compresión medular o radicular. Se comenta sobre la patología y morfología: la neuropatía mielomatosa, la discutida «toxina» mielomatosa, complicaciones vasculares y alteraciones del líquido cefalorraquídeo. El mieloma nunca invade de por sí el tejido nervioso, por lo que los síntomas nerviosos centrales se atribuyen siempre a un efecto indirecto, generalmente a una presión externa del tumor. Es importante conocer que las alteraciones neurológicas pudieran ser los primeros síntomas de una mielosis múltiple, cuya generalización se desarrolle posteriormente. Solo en los síntomas espinales del mieloma se puede conseguir una regresión pasajera merced a una operación o irradiación, no habiendo, por lo demás, tratamiento eficáz alguno.
