Die Gastropexie als alleiniger Eingriff bei Hiatushernien

Zusammenfassung

Die Anheftung des Magens an die vordere Bauchwand (Gastropexie) wird für gewisse Formen von paraoesophagealer Zwerchfellhernie empfohlen: 1. bei Incarceration, wenn wegen Alters und schlechtem Allgemeinzustand eine Kontraindikation gegen radikalere Eingriffe besteht und 2. wenn die Zwerchfellhernie ein zufälliger oder nebensächlicher Befund ist bei Kranken, die wegen einer anderen intestinalen Erkrankung operiert werden müssen. Man darf auch erwägen, ob nicht die Gastropexie die Methode der Wahl für alle Formen von Zwerchfellhernien werden kann. Zu früheren Beobachtungen.— zwei 70 Jahre alte Patienten mit inkarzerierter Zwerchfellhernie, seit 10 bzw. 6 Jahren rezidivfrei — sind zwei weitere gekommen: eine 80jährige Frau mit inkarzerierter Hiatushernie und ein 62 Jahre alter Mann mit einem Magenkarzinom und paraoesophagealer Hernie.

Summary

In this method, the hernial sack and opening are not touched. The stomach is simply attached to the anterior abdominal wall. At the beginning, the only indication for the use of this technique was incarceration of stomach and colon when the patient's general condition prohibited more formidable types of operation. A second indication has now been added, namely the incidental discovery of a diaphragmatic hernia during laparotomy performed for another purpose. Gastropexy may, in fact, prove to be the method of choice in all cases of hiatus hernia. The first two of the author's patients so treated (both 70 years-old) have now been free of recurrence and symptoms for 6 and 10 years, respectively. Two more such cases are now added. One was an 80 year-old woman with an incarcerated hernia; the other a 62 year-old man with gastric carcinoma and para-oesophageal hernia. In both instances, the operation has been a success.

La gastropexia como única intervención en la hernia del hiato

En la gastropexia permanecen, tanto el saco como el orificio herniario, incólumes, fijándose únicamente el estómago a la pared abdominal anterior. Esta intervención se empleó, al principio, en la incarceración del estómago y del intestino grueso, cuando el estado general prohibía una operación más a fondo. Como segunda indicación y por motivo de una laparotomía realizada por otros motivos, sirve una casualmente encontrada hernia diafragmática. Debe pensarse que la gastropexia puede ser el método de elección para todas las formas de hernias del hiato. A las dos primeras observaciones — dos pacientes de 70 años con hernia diafragmática incarcerada desde 10 resp. 6 años, libres de recidivas —, se han unido otros dos más: una mujer de 80 años con hernia incarcerada y un hombre de 62 años con un carcinoma gástrico y hernia paraesofágica. La intervención fué también en estos dos casos un éxito.
