Zusammenfassung

In der Regulation des Wasserhaushaltes spielt das Leberparenchym eine wichtige Rolle. Sofern andere Ursachen, z. B. renale oder endokrine Faktoren, ausgeschlossen werden können, erlaubt die Bestimmung der stündlich ausgeschiedenen Harnmenge und ihres spezifischen Gewichtes Rückschlüsse auf die Leberfunktion. Der Patient trinkt dabei stündlich 150 ccm Tee und hält Bettruhe. Erfahrungen bei 200 Leberkranken führten zur Aufstellung von 4 Diurese- und Konzentrationstypen. Die Ergebnisse entsprechen am ehesten den durch die Galaktoseprobe ermittelten Befunden. Es werden Beziehungen zum Kohlenhydrat- und Fettstoffwechsel der Leber vermutet.

Summary

The authors tested the effect of the 6-hour water test of Kauffmann-Wollheim on 200 patients with liver disease of varying degrees of severity. In 50 patients, repeated tests were performed over a period of several days to weeks. The test consists of drinking 150 cc of weak tea hourly for 6 hours and measuring amount and specific gravity of the hourly collected urine during the test period. Four types of response could be distinguished on the basis of (a) time of peak diuresis; (b) value of maximal urinary dilution; and (c) total urinary output during the first and second 3-hour periods. — Response to this test was tested in various forms of hepatic disease: healing hepatitis; mild parenchymal damage due to gallbladder or pancreatic disease; chronic damage of cirrhosis; parenchymal damage due to passive congestion of congestive heart failure; and diabetic fatty liver. For each form a rather characteristic diuretic response was observed in the majority of patients of each group. — The authors believe that a correlation between liver function, carbohydrate and fat metabolism, and water balance may be of great diagnostic importance and may be revealed by combining the 6-hour water test with the galactose tolerance test. In this manner, conclusions about the functional state of the liver in intermediary metabolism may be possible.

Los tipos de diuresis en las afecciones hepáticas

En la regulación del balance hídrico juega un gran papel el parénquima hepático. Siempre que puedan eliminarse otras causas, por ej. factores renales o endocrinos, la determinación de la cantidad de orina que se elimina por hora y su peso específico permite sacar deducciones acerca de la función hepática. Para esto el paciente bebe 150 c.c. de té por hora y guarda reposo en cama. De la experiencia en 200 hepáticos estudiados se obtienen 4 tipos de diuresis y de concentración. Los resultados corresponden muy bien con los hallazgos obtenidos con la prueba de la galactosa. Se supone que existen ciertas relaciones entre el metabolismo de los glicidos y el de las grasas.
