Zusammenfassung

Bei 115 Operationen am geöffneten und mit Azetylcholin stillgelegten Herzen werden während der Dauer der Asystole die Funktion der Lunge und des Herzens vom Pump-Oxygenator nach De Wall übernommen. Auf gesteuerte Hypothermie kann verzichtet werden. Selbst nach ausgedehntem Gebrauch ändert der Apparat Sauerstoff- und Kohlendioxyd-Spannung, pH und wichtige Elektrolyte des Blutes überraschend wenig. Unter den Patienten waren ein sieben Wochen alter, 3 kg schwerer Säugling (totale Transposition der großen Gefäße), Kinder und Erwachsene; in einem Fall wurde eine Resektion und Transplantation eines perforierten luischen Aneurysmas des Aortenbogens unter Einbeziehung der rechten Arteria carotis vorgenommen. 106 Patienten (92,2%) waren Kinder unter 16 Jahren, wovon 27 (25,5%) das erste Lebensjahr noch nicht vollendet hatten. Weitere 57 Kinder (49%) waren 1 bis 5 Jahre alt. Bei einem erwachsenen Patienten und 74 Kindern (zusammen 65% aller Fälle) wurde ein Kammerscheidewanddefekt geschlossen, welcher in 18 Fällen durch zusätzliche kardiovaskuläre Mißbildungen kompliziert war. Einfache Vorhofdefekte werden nach der „geschlossenen” Methode behandelt, bei der eine gebogene Nadel mit beidendiger Spitze unter Leitung eines durch das rechte Herzohr eingeführten Fingers benutzt wird. Der Herz-Lungen-Apparat wurde ferner in 12 Fällen angewendet, in denen wegen eines großen persistierenden Ostium primum mit ausgedehnten plastischen Eingriffen zu rechnen war. Die restlichen 28 Patienten wiesen 20 verschiedene Arten oder Kombinationen von Mißbildungen auf. Die hohe Gesamtmortalität, nämlich 45 von 115 Fällen oder 39,1%, erklärt sich zum größten Teil aus dem schlechten Zustand der Patienten vor dem Eingriff. Von 75 Fällen mit Kammerscheidewanddefekten haben wir 21 (28%) verloren, die Mortalität nimmt mit zunehmendem Alter ab. Unter den Komplikationen stehen totaler Herzblock und Kammerflimmern an erster Stelle. Von 33 Patienten mit atrio-ventrikulärer Dissoziation starben 11, weil dieser Zustand weder spontan endete noch medikamentös beeinflußt werden konnte. Unsere Serie enthält 22 Fälle von Kammerflimmern; 11 dieser Patienten starben. Während in 2 Fällen eine spontane Beendigung des Flimmerns beobachtet wurde, war in 9 anderen eine Elektroschockbehandlung erfolgreich. 2 Patienten litten an vorübergehender postoperativer Hemiplegie. Kurzfristige zerebrale Anfälle traten gelegentlich auf; sichere Rückschlüsse auf Art und Ursache dieser Störungen lassen sich heute noch nicht ziehen. Der Anaesthesist ist verantwortlich für eine reibungslose endotracheale Narkose und eine genaue Bilanz des Blut- und Flüssigkeitsvolumens. Störungen des Säure-Basen-Gleichgewichts können oft schon während der Operation erkannt und behandelt werden. Die rektale Temperatur wird kontinuierlich beobachtet und nötigenfalls korrigiert. Laufende Kontrolle der Elektrokardiogramm- und Elektroenzephalogramm-Kurven auf Leuchtschirmen erlaubt die sofortige Erkennung einer Rhythmus- und Reizleitungsstörung, der Tiefe der Anaesthesie und des Sauerstoffangebotes zum Gehirn. In zahlreichen Fällen werden zudem Druckmessungen in verschiedenen Abschnitten des kardiovaskulären Systems vorgenommen. Obwohl mit wachsender Erfahrung und verbesserter operativer Technik günstigere Ergebnisse zu verzeichnen sind, ist man sich darüber im klaren, daß viele Fragen auf dem Gebiete der extrakorporalen Zirkulation heute noch nicht beantwortet werden können.

Summary

This report is on a series of 115 open heart operations with induced cardiac arrest by Acetylcholine-injection. During the artificial asystole, a De Wall Pump-Oxygenator takes over the functions of the heart and lungs. No induced hypothermia is used. The present day use of the heart-lung-machine is based mainly on the following previous achievements: The introduction of hypothermic anesthesia, the selective perfusion of essential organs with arterialized blood while occluding the general circulation, the utilization of “Cross-Circulation” directly from a healthy donor to the patient, the designing of clinically useful heart-lung-machines, and finally, the interruption of spontaneous heart action — which further reduces the oxygen-consumption of the myocardium — by injections of cardioplegic agents as Potassium of Acetylcholine. The mechanism of the De Wall Pump is explained briefly. This machine, if used with flowrates from 50 to 75 cc/Kg/min., changes the physical and chemical properties of human blood surprisingly little, even if used for prolonged periods of time, as has been shown by determining oxygen- and carbon dioxide tensions, the pH and the more important electrolytes. The patients range from a seven week old infant, weight 3 kg., with the diagnosis of a total transposition of the great vessels, to an adult, in whom a resection and graft of a ruptured aneurysm of the aortic arch and the right carotid artery was performed. 106 patients (92.2%) were children under 16 years of age, 27 of these (25.5%) not yet one year old. 57 children were of the age from one to five years. One adult and 74 children (together 65% of all cases) had interventricular septal defects, in 18 cases complicated by the presence of additional cardiovascular anomalies. Interatrial Septal Defects of the “Secundum-Type” are treated by the closed technique, using a doublepointed curved needle under guidance of a finger within the right atrium (Conrad Lam, 1955). The pump oxygenator was used for 12 cases of Persistent Ostium Primum, in which an extensive plastic procedure had to be performed. The remaining 28 patients were operated on for 20 different types or combinations of cardiovascular anomalies. The overall mortality is 45 out of 115 cases or 39.1%, mainly due to the reduced preoperative condition of most of our patients. Of 75 patients with Interventricular Septal Defects, we have lost 21 or 28%, the mortality declining with increasing age. Total heartblock and ventricular fibrillation have been the most common complications. 33 patients experienced atrioventricular dissociation. 11 of these died, while in the remaining 22 cases either spontaneous or drug-induced conversion to sinus rhythm took place. Ventricular fibrillation occured in 22 cases and proved irreversible in 11 cases. Nine times electrical defibrillation was successful, spontaneous cessation of fibrillation was seen twice. There were two cases of transient hemiplegia. A number of patients showed symptoms of cerebral congestion, the cause and nature of which are not yet fully understood. The role of the anesthesiologist within the operating team is of increasing significance. He is responsible for the administration of a smooth endotracheal anesthesia, for the maintenance of blood-and fluid volume and in part of the acid-base balance. The rectal temperature is checked and — if necessary — corrected. Continuous observation of the EKG and EEG gives instant information about disturbances of cardiac rhythm and conduction, of depth of anesthesia and of oxygen-supply to the brain. In numerous cases the pressure in various compartments of the cardiovascular system is measured simultaneously. With growing experience and improved operative technique better results have been achieved. But it is realized, that many problems in the field of Extracorporeal Circulation are yet to be solved.

El aparato cardio-pulmonar

El oxigenador de bomba según De Wall se encarga de las funciones del pulmón y corazón durante la asistolia en 115 operaciones del corazón abierto e inmovilizado por acetilcolina. Se puede renunciar a la hipotermia dirigida. Es sorprendente la alteración insignificante que el aparato experimenta, incluso después de un uso, extenso, en cuanto a la tensión del oxígeno y dióxido de carbono, pH y electrólitos importantes de la sangre. Entre los enfermos se encontraban un lactante de 7 semanas y 3 kilos (transposición total de los vasos grandes), niños y adultos; en un caso se practicó la resección y transplantación de un aneurisma luético perforado del cayado aórtico con inclusión de la carótida derecha. 106 enfermos (92,2%) eran niños de menos de 16 años; 27 (25,5%) de estos no habían cumplido aun el año. 57 niños (49%) tenían entre 1 y 5 años. En un adulto y 74 niños (en total 65% de todos los casos) se cerró un defecto del tabique interventricular, complicado en 18 casos por añadidura de malformaciones cardiovasculares. Defectos sencillos de la aurícula son tratados con arreglo al método «cerrado», utilizándose una aguja curva con punta bilateral, dirigida por un dedo introducido a través de la orejuela derecha del corazón. El aparato cardio-pulmonar fué empleado además en 12 casos, en los cuales era de contar con extensas intervenciones plásticas por causa de un gran «ostium primum» persistente. Los 28 enfermos restantes ostentaban 20 clases o combinaciones diferentes de malformaciones. La elevada mortalidad total (45 de 115 casos = 39,1%) se explica en su mayor parte por el mal estado de los enfermos antes de la intervención. Entre 75 casos con defectos del tabique interventricular hemos perdido 21 (28%), disminuyendo la mortalidad según va aumentando la edad. Entre las complicaciones ocupan el primer lugar el bloqueo cardíaco total y la fibrilación ventricular. De 33 enfermos con disociación atrio-ventricular murieron 11, porque tal estado ni tuvo terminación espontánea ni pudo ser influido por medio de medicamentos. Nuestra serie contiene 22 casos de fibrilación ventricular muriendo 11 de estos enfermos. Mientras que en 2 casos se observó una terminación espontánea de la fibrilación, se logró un éxito en otros 9, gracias al tratamiento de choque eléctrico. 2 enfermos padecían de hemiplejía postoperatoria pasajera. Accesos cerebrales breves aparecieron ocasionalmente; de momento no es posible todavía sacar conclusiones seguras en cuanto a la índole y causa de tales trastornos. El anestesista es responsable de la narcosis endotraqueal eficaz y del balance preciso del volumen de sangre y líquido. Alteraciones del equilibrio ácido-básico pueden a menudo ser reconocidas y tratadas ya durante la operación. La temperatura rectal es observada continuamente, siendo corregida en caso necesario. El control constante de las curvas del electrocardiograma y electroencefalograma por medio de pantallas luminosas permite reconocer inmediatamente un trastorno del ritmo y de la transmisión de estímulos, profundidad de la anestesia y oferta de oxígeno al cerebro. En numerosos casos se practican además mediciones de la presión en diferentes sectores del sistema cardiovascular. Aunque la experiencia creciente y el perfeccionamiento de la técnica operatoria permiten registrar resultados más favorables, se ve con claridad que muchas cuestiones de la circulación extracorporal no pueden ser contestadas todavía en la actualidad.
