Zusammenfassung

Es wird das Krankheitsbild der Urticaria pigmentosa als generalisierte, genuine Mastocytose beschrieben. Die klinische Symptomatik besteht bei voller Ausprägung aus charakteristischen Hauterscheinungen, Lymphadenopathie bzw. Leber- und Milztumor, Skelettveränderungen und gastroenteritischen Symptomen. Pathogenetisch ist die Erkrankung durch eine allgemeine Mastzellenhyperplasie gekennzeichnet, wobei Zellinfiltrate in den subbasalen Schichten der Haut, in Lymphknoten, Leber und Milz und im Knochenmark auftreten. Die Funktion der Mastzellen, Histamin und Heparin zu enthalten und z. T. zu bilden, charakterisiert weitere Besonderheiten der Krankheit Während die Histaminwirkung die urtikariellen Hauterscheinungen, die Kapillarveränderungen und die gastroenteritischen Symptome erklären läßt, werden Gerinnungsstörungen und Abweichungen im Lipoproteidstoffwechsel als Folge der Hyperheparinämie beschrieben, Auf die nosologische Stellung des Krankheitsbildes wird eingegangen und die Mastocytose anderen Systemerkrankungen mesenchymaler und blutbildender Gewebe an die Seite gestellt.

Summary

The clinical and laboratory findings in generalised mast-cell disease are described. In its full-blown stage there are characteristic skin changes, lymphadenopathy, hepato- and splenomegaly, skeletal changes and gastroenteritic symptoms. The disease is caused by a generalised mast-cell hyperplasia with cellular infiltration into the sub-basal layers of the skin, into the lymph nodes, liver spleen and bone marrow. Many of the typical features of the disease are explainable on the basis of mast-cell function, i.e. formation and storage of histamine and heparin. Thus histamine is responsible for the urticarial signs, the capillary changes and the gastro-enteritic symptoms, while changes in coagulation and abnormalities of lipoprotein metabolism are considered the consequence of an hyperheparinaemia. This disease belongs to the group of systemic involvement of mesenchymal and haematopoietic tissues.

Resumen

La mastocitosis

Se describe el cuadro nosologico de la urticaria pigmentosa como mastocitosis genuina generalizada. La sintomatologia clinica consiste, cuando es completa, en manifestaciones cutaneas caracteristicas, adenopatias, hepato y esplenomegalia, alteraciones esqueleticas y sintomas gastroentericos. La enfermedad se caracteriza patogenicamente por una hiperplasia generalizada de las celulas cebadas, mastocitos, con aparicion de infiltrados celulares en las capas subbasales de la piel, en los ganglios linfaticos, higado, bazo y medula osea. La funcion de los mastocitos, de almacenar histamina y heparina y formarlas en parte, explica otras particularidades de la afeccion. Mientras que la accion de la histamina permite explicar las manifestaciones cutaneas urticariformes, las alteraciones capilares y los sintomas gastroentericos, se describen las alteraciones de la coagulacion y los trastornos del metabolismo lipoideo como consecuencias de una hiperheparinemia. Se estudia la situacion nosologica del cuadro clinico y se equipara la mastocitosis a otras afecciones sistematizadas de tejidos mesenquimales y hematopoyeticos.
