Zusammenfassung

Bei einem 72-jährigen Patienten mit einer bekannten chronischen lymphatischen Leukämie entwickelten sich insektenstichartige Hautveränderungen am Stamm und an den Extremitäten. Diese Läsionen, die nicht auf tatsächlich stattgefundene Insektenstiche zurückzuführen sind, lassen sich weder klinisch noch histopathologisch von echten Insektenstichreaktionen unterscheiden. Für die Differenzialdiagnose entscheidend sind die Anamnese, die Verteilung der Läsionen, der Verlauf der Dermatose, das Versagen präventiver Maßnahmen gegen vermeintlich vorhandene Insekten und die ebenfalls fehlende Effektivität durchgeführter Therapieverfahren. Da die insektenstichartigen Hautveränderungen, von wenigen Ausnahmen abgesehen, hauptsächlich bei Patienten mit Leukämien, seltener mit Lymphomen beobachtet werden, muss beim Auftreten der Dermatose immer eine weiterführende internistische Diagnostik veranlasst werden. Die insektenstichartigen Hautveränderungen erfüllen im Übrigen die Kriterien einer kutanen Paraneoplasie.

Abstract

A 72-year old patient with chronic lymphocytic leukemia developed insect bite-like lesions on the trunk and the limbs. These insect bite-like skin lesions, which are not caused by real insect bites, can not be differentiated from real insect bites on the basis of clinical or histopathological findings.

Important for the differential diagnosis are the medical history, the distribution of the lesions, the progression of the lesions, the failure of preventive measures against putative existing insects and the lack of efficacy of previously applied therapies. It is important to carry out further internal diagnostic investigations, even if insect-bite like skin reactions, except for the occasional ones, mostly occur in patients with leukemia rather than in patients with lymphoma. Insect bite-like skin lesions fulfil the criteria for cutaneous paraneoplasms.
