Entwicklung der Chest-Pain-Units in Deutschland

Abstract

Clinical studies in the US and UK clearly demonstrate that the establishment of a chest pain unit (CPU) improves the prognosis of patients with chest pain and myocardial infarction and also helps to reduce costs. To guarantee a systematic establishment of CPUs in Germany, the German Society of Cardiology (DGK) founded in 2007 a „task force chest pain unit” to develop nationally valid minimum standards for a chest pain unit (prerequisites for a CPU certification program to evaluate CPUs across the country). In 2008 recommendations for the necessary minimum standard of a CPU were published for the first time in Germany. At the moment 26CUPs are certified in Germany. The aim of the DGK is to certify around 300 centres/units throughout Germany within the next 2 years in order to provide nationwide care of patients with acute thoracic pain.

In Mainz, a close cooperation with private cardiologists within the University Medical Center (Universitätsmedizin) provides an ideal interface for the treatment of outpatients and stationary patients with acute chest pain. Furthermore, public awareness campaigns with the soccer club Mainz 05 help to inform the population about the availability of this new structure and how to react quickly in the case of chest pain. The building-up of chest pain units accompanied by the certification process and awareness campaigns will optimize education and treatment of patients with chest pain.

The German society of cardiology highlights with their campaigns the importance of CPUs with regard to the treament of patients with acute chest pain in Germany and offers great potential for further developments and innovations within this new structure.

  • Für die Versorgung von Patienten mit akutem Brustschmerz sind eine kurze Prähospitalzeit und eine optimale Diagnostik und Therapie erforderlich.

  • Durch die Einrichtung einer CPU wird die Prognose von Patienten mit einem Herzinfarkt verbessert und es werden finanzielle Ressourcen eingespart. Optimale Diagnostik und Therapie sind in einer CPU gewährleistet.

  • Die Deutsche Gesellschaft für Kardiologie (DGK) hat in einer Arbeitsgruppe einen national verbindlichen Mindeststandard einer CPU definiert und zertifiziert CPUs auf Antrag.

  • Aktuell sind in Deutschland 26 CPUs zertifiziert. In den nächsten 2 Jahren sollen mithilfe der DGK insgesamt etwa 300 Zentren deutschlandweit zertifiziert werden, um eine flächendeckende Versorgung von Patienten mit akutem Brustschmerz zu erreichen.

  • In Mainz werden „Awareness”-Kampagnen mit dem Fußballverein Mainz 05 eingeleitet, um für einen höheren Bekanntheitsgrad der CPUs zu sorgen. Dies wird als wichtiger Faktor gesehen, um eventuelle Hemmungen, eine Notrufnummer zu wählen, abzubauen und damit die Prähospitalzeit zu verkürzen.
