Gastrointestinale Blutung

  • Die obere gastrointestinale Blutung stellt unverändert einen der häufigsten medizinischen Notfälle dar. Der ideale Zeitpunkt der Endoskopie wird kontrovers diskutiert. Trotzdem gilt bisher die Empfehlung, die Endoskopie sofort durchzuführen, insbesondere bei Verdacht auf eine Varizenblutung.

  • Beim Verdacht auf eine Varizenblutung sollten bereits vor der Endoskopie Octreotid oder Terlipressin und bei Verdacht auf Blutungen anderer Genese ein Protonenpumpenhemmer verabreicht werden.

  • Durch eine antibiotische Prophylaxe bei Varizenblutung können Infektionsrate, Rezidivblutungsrate und Transfusionsbedarf gesenkt werden.

  • Die gezielte Applikation eines Hämoclips auf einen blutenden Gefäßstumpf ist die sicherste Art der endoskopischen Blutstillung. Nur wenn eine Clipsetzung durch schlechte Sichtverhältnisse bei starker Blutung behindert wird, erscheint eine vorherige Injektionstherapie sinnvoll.

  • Bei der Ösophagusvarizenblutung hat die Ligaturtherapie in den letzten Jahren die Sklerotherapie wegen ihres besseren Sicherheitsprofils und ihrer größeren Effektivität als Erstlinientherapie abgelöst.

  • In der Regel verlaufen Blutungen aus dem Kolon nicht fulminant, sodass die Möglichkeit einer Darmlavage besteht. In allen Fällen einer massiven Hämatochezie und insbesondere bei instabilen Patienten sollte zusätzlich eine Ösophagogastroduodenoskopie zum Ausschluss einer massiven Blutung im oberen Verdauungstrakt durchgeführt werden.

Akute Cholangitis und biliäre Pankreatitis

  • Bei der akuten Cholangitis ist neben der antibiotischen Therapie die notfallmäßige Drainage durch eine Endoprothese oder eine nasobiliäre Sonde erforderlich.

  • Bei der biliären Pankreatitis ist eine frühe ERCP vor allem bei Patienten mit Zeichen der Gallengangobstruktion und/oder Cholangitis sinnvoll.

Fremdkörper

  • Im Zweifelsfall sollten bei einer möglichen Fremdkörperingestion eine Röntgenaufnahme des Thorax und des Abdomens zur Lokalisation durchgeführt werden, da vermeintlich verschluckte Objekte auch aspiriert worden sein können.

Maligne Obstruktion

  • Bei einem Dickdarmileus im Rahmen einer malignen Obstruktion ist die Einlage eines selbstexpandierenden Metallstents als „bridge to surgery” sicher und effektiv. Die Anlage eines protektiven oder permanenten Stomas kann in vielen Fällen vermieden werden.

Iatrogene Perforation

  • Nicht alle iatrogenen Perforationen müssen heute operativ versorgt werden. Ein Verschluss mit Hämoclips oder neueren Verschlusstechniken ist möglich und oftmals erfolgreich.
