Schlank trotz genetischer Prädisposition für Adipositas – Beeinflussung von Umweltfaktoren als Chance? Eine Kasuistik

Zusammenfassung
Anamnese und klinischer Befund: Ein 4 Jahre altes, leicht übergewichtiges (BMI 18 kg/m2, BMI SDS 1,29 ∼ 90,1. Perzentile), aber sonst gesundes Kind wurde wegen Hochwuchs (99,6. Perzentile) und permanentem Essdrang/fehlendem Sättigungsgefühl vorgestellt. Diese Symptomatik ist vereinbar mit Melanocortin-4-Rezeptor(MC4R)-Mutationen.
Genetische Untersuchung und Diagnose: Durch Sequenzierung des kodierendes Exons des MC4R wurde die Mutation Arg165Trp nachgewiesen, welche die Rezeptorfunktion partiell inaktiviert. Die Mutter ist wie das Kind heterozygote Trägerin dieser Mutation und interessanterweise schlank (BMI 19 kg/m2).
Therapie und Verlauf: Die Prädisposition für Adipositas resultiert bei Trägern von funktionsbeeinträchtigenden MC4R -Mutationen aus der Hyperphagie infolge der fehlenden Regulation des Sättigungsgefühls. Es existiert keine kausale Therapie, sie muss vielmehr am Lebensstil ansetzen. Durch frühzeitige Diagnose konnten die Eltern über eine Restriktion der Nahrungsaufnahme und Förderung der körperlichen Aktivität den Gewichtsverlauf bisher positiv beeinflussen.
Folgerung: Auch bei genetischer Prädisposition können Lebensstil und Umweltfaktoren positive Effekte auf das Körpergewicht ausüben.
Summary
Case history and clinical findings: A 4-year-old, slightly overweight (BMI 18 kg/m2, BMI SDS 1.29 ∼ 90.1th percentile) and otherwise healthy girl presented with accelerated linear growth (99.6th percentile) and hyperphagia due to loss of satiety feeling. These findings are consistent with melanocortin-4 receptor (MC4R) mutations.
Genetic analysis and diagnosis: We found the partially inactivating mutation Arg165Trp by direct sequencing of the MC4R coding region. Interestingly, the patient’s mother is also heterozygous for this mutation, but lean (BMI 19 kg/m2).
Treatment and course: Carriers of MC4R mutations develop hyperphagia due to lack of satiety feeling as a result of central dysregulation. The reduction of energy intake and the encouragement of physical activity can be successful to control the body weight.
Conclusion: Early diagnosis can promote lifestyle intervention to prevent the development of obesity even in the presence of a genetic predisposition.
