Zusammenfassung

Berichtet wird der Fall einer 48-jährigen, depressiven Patientin mit kardiogenem Schock und breiten QRS-Komplexen. In der Annahme einer Intoxikation mit einem Klasse-I-Antiarrhythmikum in suizidaler Absicht wurde die Patientin erfolgreich kardiopulmonal reanimiert. Neben der üblichen Medikation erfolgte die Gabe von hypertoner Kochsalzlösung und Natriumbikarbonat. Eine Flecainidintoxikation ließ sich retrospektiv durch erhöhte Serumspiegel bestätigen. Bei Reanimationssituationen und breiten QRS-Komplexen sollte auch an eine Überdosierung / Intoxikation mit Klasse-I-Antiarrhythmika gedacht werden. Im Falle einer unzureichenden Stabilisierung kann die zusätzliche Gabe von hypertoner Kochsalzlösung und / oder Natriumbikarbonat hilfreich sein.

Abstract

The 48-year-old woman was found unresponsive and hypotensive with broad QRS, assuming an attempted suicide with Class I antiarrhythmic drugs. Cardiopulmonary resuscitation and advanced life support were sustained for 90 minutes. Following intravenous hypertonic saline and sodium bicarbonate the patients QRS narrowed and haemodynamics could be restored. Flecainide intoxication was approved by elevated serum level. The patient survived without neurological sequelae. Intoxication with Class I antiarrhythmic drugs should be considered in cases of cardiogenic shock with broad QRS-ecg. Additional treatment with hypertonic saline and / or sodium bicarbonate could help to stabilize heart rhythm and haemodynamics.
