Zusammenfassung
Die Differentialdiagnose zwischen Karzinom und entzündlichem Prozeß im Lungenmantel ist mit Hilfe des Bronchogramms in vielen Fällen möglich. Sie stützt sich auf das obligate Vorhandensein einer deformierenden Bronchitis beim entzündlichen Prozeß und auf ihr Fehlen beim Karzinom im Lungenmantel. Dagegen sind Bronchialverdrängungen, Stenosen und Verschlüsse allein weder für ein Karzinom im Lungenmantel beweisend, noch schließen sie einen entzündlichen Lungenprozeß aus.
Summary
Differentiation between carcinomas and inflammatory lesions in the lung cortex is possible in many cases with the aid of a bronchogram. This depends on the invariable presence of deforming bronchitis in inflammatory lesions and its absence in cases of carcinoma of the lung cortex. On the other hand, displacement, stenosis or occlusion of bronchi does not prove the existence of a carcinoma in the lung cortex nor does it exclude an inflammatory pulmonary process.
Résumé
Le diagnostic différentiel entre un cancer et une affection inflammatoire du «manteau» pulmonaire est possible dans beaucoup de cas grâce à la bronchographie. Elle repose sur le fait qu'une inflammation détermine nécessairement une bronchite déformante, ce qui n'est pas le cas avec le cancer périphérique. La seule présence de refoulement bronchique, de sténose et d'obstruction ne constitue pas une preuve de l'existence d'un cancer périphérique et ne permet pas d'exclure une affection pulmonaire inflammatoire.
Resumen
Con el auxilio del broncograma es posible, en muchos casos, el diagnóstico diferencial entre carcinoma y proceso inflamatorio en el manto pulmonar. Se basa en la presencia obligada de una bronquitis deformante en los procesos de carácter inflamatorio y en su ausencia en el carcinoma del manto pulmonar. Por el contrario determinadas dislocaciones bronquiales, las estenosis y oclusiones, no son demostrativas, por sí solas, para un carcinoma del manto pulmonar ni tampoco excluyen un proceso pulmonar inflamatorio.
