Isolierungsmaßnahmen in der Intensivmedizin[1]

Kernaussagen

• Grundlegende Standardhygienemaßnahmen gelten bei jedem Patienten und dienen dazu, Übertragungen von Erregern zu verhindern.

• Bei bestimmten Infektionskrankheiten bzw. Erregern werden entsprechend dem Übertragungsmodus zusätzliche infektionspräventive Maßnahmen empfohlen (z. B. bei der aerogenen Übertragung der Tuberkulose).

• Zusätzliche, d. h. übertragungsspezifische Maßnahmen sind Kontaktisolierung, Tröpfchenisolierung und aerogene Isolierung.

• Auch multiresistente Erreger wie MRSA, VRE und ESBL werden in der Regel vor allem durch Hände/Handschuhe des medizinischen Personals übertragen.

• Bei der Übertragungsprävention hat die Frage, ob die Unterbringung in einem Einzelzimmer erforderlich ist oder nicht, nicht die Bedeutung, die ihr oft zugewiesen wird. Die räumliche Abtrennung von Patienten mit multiresistenten Erregern ist lediglich ein Element in einem ganzen Katalog von Einzelmaßnahmen. Die entscheidende Maßnahme, die eine Unterbrechung der Erregerübertragung bewirken kann, ist die Händehygiene.

• Wenn Hände vor und nach Patientenkontakt nicht desinfiziert werden und Handschuhe nicht gewechselt werden, kann auch kein Einzelzimmer helfen. Ohne Händehygiene sind auch Kittel und Maske entbehrlich.

• Vom RKI gibt es Empfehlungen zum Umgang mit MRSA (Prinzip der Kontaktisolierung).
