Abstract

The Alfieri Stitch method has broadened the techniques of mitral reconstructive surgery. Preliminary results have shown acceptable short-term mortality and good functional results, mainly in patients with degenerative mitral valve disease. Long-term results, however, are unknown. One important contributor to the results of the Alfieri technique is the quality of the mitral annuloplasty performed in parallel. Main drawback is the risk of functional mitral stenosis of the double-orifice valve after Alfieri repair. The risk of endocarditis or valve fibrosis after this operation is unknown to date. In conclusion, the opinion of the authors is that, given the excellent results of current mitral reconstructive surgery according to the Carpentier method, the Alfieri stich will remain a second-line technique for complex mitral valve pathologies.
