Die Ursachen für inkomplette Blockaden, die beim N. alveolaris inferior mit 25 % und mehr angegeben werden, liegen ursächlich an anatomischen Besonderheiten und dem Gewebezustand, aber auch an der angewandten Technik bzw. der Lokalisation [4]. Die Wahrscheinlichkeit, bei der Insertion ein Gefäß zu treffen, wird mit bis zu 20 % angegeben [17], was bei Hämophiliepatienten zum Ausschluss dieser Methode der Lokalanästhesie führt. Bei Patienten mit hämorrhagischer Diathese dürfen Leitungsanästhesien auf keinen Fall vorgenommen werden, da diese infolge massiver Hämatombildung lebensbedrohliche Folgen haben können [24].

Eine unzureichende Anästhesie erfordert eine Komplettierung – häufig durch Wiederholen der Injektion. Dabei ist nicht auszuschließen, dass es zu einem Nervkontakt kommen kann, ohne eine Reaktionsmöglichkeit des Patienten, da bereits eine größere Menge Anästhetikum das Injektionsgebiet desensibilisiert hat. Eine Nervläsion – auch mit lang dauernden Folgen – ist nicht auszuschließen [11].

Incomplete nerve block anaesthesia is a daily problem in dentistry and figured to be more than 25 %. Anatomical abnormities, the state of the tissue but also the technique applied and the localisation are defined as origin [4]. The probability to hurt a vessel during the insertion of the needle is up to 20 % [17], excluding therefore this anaesthesia method for haemophilia prone patients, the formation of haematoma may have life-threatening consequences [24].

Periodontal ligament injections of anaesthetics can be considered as an alternative to the IANB and to the infiltration anaesthesia even for patients with a chronicle periodontitis marginalis, prior to any teeth conserving therapy. The direct clinical comparison of 150 cases each point out the high success rate and the advantages for patient and operator of the intraligamentary anaesthesia (ILA), without any histological difference within the methods of local anaesthesia compared. The ILA can be considered as a primary method for the dental local anaesthesia provided adequate armamentaria are used, proved anaesthetics are applied and the method is mastered safely by the operator.

Prior to long lasting and extended surgical treatments the IANB and the infiltration anaesthesia now, as before, are the methods of choice, the intraligamentary anaesthesia not being capable to provide an adequate analgesia for these indications.
