Zusammenfassung
In einer prospektiven Erhebung wurden Jugendliche mit Typ-1-Diabetes in einer diabetologischen
Schwerpunktpraxis für Kinder und Jugendliche (N = 93; Rücklaufquote 66,7 %) zu ihren
Erwartungen und Wünschen in Bezug auf den bevorstehenden Betreuungswechsel befragt,
um Hinweise auf eine von Jugendlichen präferierte Gestaltung des Übergangs zu erhalten.
62 Jugendliche (Alter: 15,7 ± 2,1 Jahre; weiblich: 53,2 %) gaben Auskunft über den
gewünschten Zeitpunkt des Betreuungswechsels, ihre Wünsche bezüglich der Vorbereitung
auf den Wechsel und die Gestaltung der Übergabe sowie ihre Erwartungen an die weiterbetreuende
Einrichtung. Die meisten Jugendlichen (82 %) möchten zu einem individuell festgelegten
Zeitpunkt die betreuende Einrichtung wechseln, wobei sie sich einen vorbereiteten
und begleiteten Übergang von der Kinder- in die Erwachsenendiabetologie wünschen (79 %),
bei dem sie aktiv beteiligt werden (72,6 %). Ein Drittel (32,3 %) macht sich aus unterschiedlichen
Gründen Sorgen über den bevorstehenden Betreuungswechsel. Von der erwachsenendiabetologischen
Einrichtung wünscht sich die Mehrheit besondere Betreuungsangebote für die Zielgruppe
der Jugendlichen, z. B. Schulungen für Jugendliche und junge Erwachsene (82,3 %).
Mädchen denken signifikant häufiger über das Thema Transition nach als Jungen (p ≤ 0,05);
sie möchten häufiger eine aktive Rolle einnehmen (p ≤ 0,05), wünschen sich häufiger
Unterstützung seitens ihrer Familie (p ≤ 0,01) und sorgen sich häufiger als Jungen
(p ≤ 0,05). Jugendliche mit einer ungünstigeren Stoffwechseleinstellung (HbA1c ≥ 8,0 %)
würden seltener in die kinderdiabetologische Praxis zurückkehren, wenn es ihnen in
der weiterbetreuenden Praxis nicht gefällt als Jugendliche mit einem HbA1c < 8,0 %
(p ≤ 0,05).
Abstract
In a prospective assessment youths with type 1 diabetes in a diabetological ambulant
center for children and youths (N = 93; rate of return 66.7 %) were asked for their
expectations and wishes with respect to the upcoming transition, in order to get to
know their preferences for transfer arrangements. 62 youths (age: 15.7 ± 2.1 years;
female: 53.2 %) answered questions concerning preferred time for transfer, wishes
for transfer preparations and arrangements as well as expectations with respect to
the further care providing physician. Most of the youths (82 %) want to change their
health care provider at a point of time that is determined individually; they wish
to have a well prepared and accompanied transfer from pediatric to adult care (79 %),
in which they take over an active role (72.6 %). One third (32.3 %) worries about
the transfer for different reasons. The majority of youths want the adult care provider
to offer specials for youths, for example patient education groups for youths and
young adults (82.3 %). Girls reflect on the upcoming transition more frequently than
boys (p ≤ 0.05); they more often want to participate actively in transition (p ≤ 0.05),
more often want support from their families (p ≤ 0.01) and are more worried than boys
(p ≤ 0.05). Youths with poor glycemic control (HbA1c ≥ 8.0 %) would go back to the
pediatric health care provider less often, when unsatisfied with the adult care provider
than youths with good glycemic control (HbA1c < 8.0 %; p ≤ 0.05).
