Zusammenfassung

Anatomische Faktoren wie ein dünner gingivaler Biotyp, Prominenz von Zähnen oder Zahnwurzeln, eine traumatisierende Zahnputztechnik oder die orthodontische Bewegung von Zähnen aus der Alveolarplatte heraus wurden als mögliche Ursachen zur Entstehung von gingivalen Rezessionen genannt. Das Behandlungsziel beim vorliegenden Fallbericht sieht die Therapie der generalisierten Gingivitis sowie der singulären Miller-Klasse-II-Rezession an Zahn 41 nach kieferorthopädischer Therapie mit der modifizierten Tunnel-Technik vor. Die angewandte Technik der Tunnellierung kombiniert mit einem Bindegewebe-Transplantat bringt Vorteile für das Gewebe und den Patienten: Durch die auf den gingivalen Sulkus beschränkte Schnittführung und die Anwendung mikrochirurgischer Operationstechniken kann eine Narbenbildung weitgehend vermieden werden. Die Verwendung eines Bindegewebetransplantates bietet zusätzlich eine Verdickung der Gingiva. Das Behandlungsresultat 3 Monate nach Chirurgie zeigt eine signifikante Verbesserung der klinischen und ästhetischen Situation.

Abstract

Anatomical conditions such as a thin gingival biotype, prominence of teeth or root surfaces, traumatic oral hygiene techniques or the orthodontic movement of teeth out of the alveolar bone plate are discussed to be etiological factors in the development of gingival recessions.

This case report describes the treatment of a generalized gingivitis and a Miller class II gingival recession at tooth 41 after orthodontic treatment using the modified tunnel technique. This technique combined with a connective tissue graft brings many advantages to the tissues and to the patient: incisions limited to the gingival sulcus and microsurgical techniqes provide minimal scar formation, and the connective tissue graft is able to thicken the gingiva. The result obtained 3 months after surgery shows a significant clinical and esthetical improvement of the situation.
