Transarterielle Chemoembolisation von primären und sekundären Lebertumoren

Die Leber nimmt im menschlichen Stoffwechsel eine wichtige Rolle ein und ist somit Zielorgan für die Metastasierung einer Reihe von Karzinomen, insbesondere für Malignome aus dem Gastrointestinaltrakt. Sie kann aber auch Sitz primärer Lebertumoren sein. Bei einem Befall der Leber durch primäre oder sekundäre maligne Lebertumoren ist sowohl das Langzeitüberleben als auch die Lebensqualität der betroffenen Patienten entscheidend reduziert [1] [2] [3] [4]. Die Therapie von Lebermetastasen erfordert eine interdisziplinäre, multimodale Therapie. Ziel aller zur Verfügung stehenden Therapiemaßnahmen (chirurgische, onkologische und interventionell-radiologische Verfahren) ist neben einer hohen, lokalen Tumorkontrolle auch und vor allem die Verlängerung des Überlebens des Patienten.

Kernaussagen

  • Chemotherapie erfolgt soweit wie möglich am Tumor.

  • Durch den Einsatz von Embolisaten lässt sich die Wirkung wesentlich erhöhen.

  • Therapeutischer Einsatz radiologischer Interventionstechnik.

  • Für die Patienten relativ gut verträglich.

  • Superselektive Technik mit Mikrokatheter erhöht Wirkung und senkt Nebenwirkungen.

  • Minimal-invasive interventionelle Therapie als Beispiel der interdisziplinären Zusammenarbeit.

  • Weitere Interventionen, wie die Chemoembolisation mit Zytostatika beladene Partikel/Sphären, werden die Therapiemöglichkeiten für die Patienten erweitern.

  • Beispiel für die Vielseitigkeit der Arbeitsbereiche der Radiologie.
