Zusammenfassung
Mit diesem Review soll die Bedeutung der standarisierten histopathologischen Diagnostik
von Synoviliasbiopsaten und Synovektomiepräparaten für die Stratifizierung der Therapie
der Arthritiden verdeutlicht werden.
Als diagnostisches Hilfsmittel wird der Synovialitis-Score zur Unterscheidung von
low-grade (assoziiert mit degenerativen und posttraumatischen Arthropathien) und high-grade
Synovialitiden (assoziiert mit entzündlichen Erkrankungen des rheumatischen Formenkreises)
vorgestellt. Die Sensitivität für die Diagnose beträgt damit 60,5% bei einer Spezifität
von 95,5%.
Zusätzlich zur Differenzierung einer chronischen Synovialitis können histopathologisch
Aussagen zu einer infektiösen oder kristallinduzierten Synovialitis anhand der Biopsie
zuverlässig getroffen werden.
Eine wichtige Aufgabe der Histopathologie besteht im Ausschluss einer PVNS, welche
in den klinischen Diagnoseverfahren oft nicht detektiert wird.
Der Review schließt mit einem Ausblick der möglichen Bewertung des Therapieerfolges
sog. Biologika mittels Bestimmung des Remissionsgrades der Synovialitis („sog. Regressionsgradings
der Entzündung“).
Abstract
This review emphasizes the role of a standardised histopahological diagnosis of synovial
biopsies and synovectomy specimens in order to stratify the therapy of different forms
of arthritis.
The synovitis-score should be applied in these cases as a diagnostic device, leading
to the diagnosis of a low-grade synovitis (which is associated with degenerative and
posttraumatic arthropathies) or of a high-grade synovitis (associated with rheumatic
diseases), the sensitivity and specificity being 60.5% and 95.5%, respectively.
In addition, synovial biopsies could be sufficiently used for the histopathological
differentiation of infectious synovitis, crystal-induced arthropathies and the diffuse
form of the tenosynovial giant cell tumor. The diffuse form of the tenosynovial giant
cell tumor is sometimes misdiagnosed in pre-bioptic diagnostic procedures.
The review closes with an outlook about future perspectives of synovial biopsies and
synovectomy specimens for the assement of the therapeutic outcome of biological treatment
of rheumatoid arthritis via regression grading of synovitis activity.
