Pathogenese, Klinik, Diagnostik

  • Ösophagusdivertikel werden anhand der Pathogenese und der Lokalisation eingeteilt. Die häufigsten Divertikel sind das Zenker-Divertikel oberhalb des oberen Sphinkters und das epiphrenische Divertikel oberhalb des unteren Sphinkters. Überwiegend handelt es sich um Pulsionsdivertikel.

  • Die Leitsymptome des Ösophagusdivertikels sind Dysphagie und Regurgitation, aber auch atypische Symptome wie rezidivierende Aspirationspneumonien können auftreten. Diagnostisch wegweisend sind ÖGD und Bariumbreischluck.

Therapie

  • Eine Therapieindikation besteht in der Regel nur bei symptomatischen Divertikeln.

  • Zenker-Divertikel können endoskopisch therapiert werden mittels Septotomie. Die endoskopische Therapie gewinnt zunehmend an Stellenwert aufgrund der geringeren Morbidität gegenüber offen chirurgischen Verfahren.

  • Bei symptomatischen Divertikeln im Ösophagus ist die chirurgische Therapie der Standard, auch hier geht der Trend zunehmend zur minimalinvasiven Therapie.
