Vorhofflimmern (VHF) ist die häufigste Herzrhythmusstörung im Erwachsenenalter mit einer altersabhängigen Zunahme auf 5–15 % unter den über Achtzigjährigen und stellt aufgrund des deutlich erhöhten Risikos an Primär- und Sekundärkomplikationen, insbesondere dem Auftreten von Schlaganfällen, eine sozioökonomisch wichtige Erkrankung dar. VHF ist für etwa 25 % aller ischämischen Schlaganfälle verantwortlich [1] und das Schlaganfallrisiko von Patienten mit VHF ist gegenüber Patienten ohne VHF um das Vier- bis Fünffache erhöht [2]. Bei Patienten mit vorausgegangener transienter ischämischer Attacke (TIA) oder Schlaganfall trat bei 13 % pro Jahr ein erneuter Schlaganfall auf. In diesem Artikel sollen aktuelle Empfehlungen zur Diagnostik, Therapie, Primär- und Sekundärprävention gegeben werden.

Atrial fibrillation (AF) is the most common cardiac arrhythmia in the adult population. Its prevalence increases with age up to 5–15 % in patients over eighty years. It has a high risk for primary and secondary complications such as ischemic strokes or thromboembolic events. In about 25 % AF is the cause of ischemic stroke and stroke risk is four to five-fold higher in patients with atrial fibrillation compared to those without AF. In this article we would give current recommendations for diagnostic, therapy, primary and secondary prophylaxis.
