Zusammenfassung

Unfälle sind weiterhin die häufigste Todesursache bei Kindern und jungen Erwachsenen. Das Management polytraumatisierter Patienten hat sich in den letzten Jahren durch Qualitätsoffensiven (Traumanetzwerk, S3-Leitlinie Polytrauma) verbessert. Auf dieser Grundlage sind die gute Vernetzung zur Präklinik, strukturierte Behandlungsalgorithmen, Ausbildungsstandards (ATLS®), klare Regeln der Diagnostik und ein Qualitäts- und Risikomanagement die Faktoren einer modernen Schockraumversorgung. Wir beschreiben diese organisatorischen Bausteine, die zu einem erfolgreichen Traumamanagement in der Klinik führen.

Abstract

Injuries remain the leading cause of death in children and young adults. Management of multiple trauma patients has improved in recent years by quality initiatives (trauma network, S3 guideline “Polytrauma”). On this basis, strong links with preclinical management, structured treatment algorithms, training standards (ATLS®), clear diagnostic rules and an established risk- and quality management are the important factors of a modern emergency room trauma care. We describe the organizational components that lead to successful management of trauma in hospital.

  • Die S3-Leitlinie Polytrauma informiert seit 2011 umfassend über die Entscheidungsfindung in typischen Versorgungssituationen.

  • Eine optimale Kommunikation zwischen Präklinik und Schockraumteam sichert einen zügigen und vollständigen Informationsaustausch.

  • Traumanetzwerke ermöglichen die Auswahl eines geeigneten Zielkrankenhauses und garantieren die Aufnahme polytraumatisierter Patienten.

  • Durch standardisierte Ausbildungskonzepte (ATLS®) hat sich eine gemeinsame „Schockraumsprache“ entwickelt.

  • Versorgungsfehler mit tödlichen Folgen treten auch in erfahrenen Traumazentren auf.

  • Ein effektives Risiko- und Fehlermanagement ermöglicht das Lernen aus eigenen Versorgungsfehlern.

  • Die Umsetzung der Leitlinien in die tägliche klinische Praxis erfordert großes Engagement und gute Absprachen der vor Ort beteiligten Fachabteilungen.

  • Berufs- und fachgruppenübergreifende Simulations- und Trainingsmaßnahmen können Abläufe und Kommunikation verbessern.
