Zusammenfassung
Hintergrund:
Die Tonsillektomie wird international stationär wie auch ambulant durchgeführt. Vor
dem Druck der Kostenreduzierung im deutschen Gesundheitssystem stellt sich die Frage
nach dem ökonomischen Nutzen bei einer ambulanten Durchführung.
Material und Methoden:
Es erfolgt das Herleiten von Vergleichsformeln und anschließende Berechnung der Vergütungen
bzw. Kosten einer ambulanten sowie stationären Tonsillektomie für die Leistungserbringer
bzw. Kostenerstatter. Es finden die Vergütungssysteme nach DRG sowie EBM Anwendung.
Ergebnisse:
Die ambulante Durchführung der (Adeno-)Tonsillektomie kann in Deutschland eine jährliche
Einsparung von 213,5 Millionen Euro für die Kostenerstatter erbringen. Für die Leistungserbringer
wird die stationäre Durchführung empfohlen.
Schlussfolgerung:
Unter der Berücksichtigung der gesamtwirtschaftlichen Kosten und der medizinischen
Behandlungsqualität wird insgesamt die stationäre Durchführung empfohlen.

Inpatient vs. Outpatient Costs Analysis of the Tonsillectomy
Background:
The tonsillectomy is realized inpatient and outpatient in different countries. Caused
by the pressure of reduction of expenses there is the question of the economic benefit
if done outpatient in Germany.
Material and Methods:
A comparison of the inpatient and outpatient gratification will be done.
Results:
There is a yearly potential of cost reduction of 213.5 million euro, if the operation
would be done as an outpatient procedure. From the hospital view there is no economical
recommendation doing an outpatient tonsillectomy.
Conclusion:
With simultaneous consideration of economical reasons, the medical quality and macroeconomic
reasons there is no recommendation doing the tonsillectomy as an outpatient operation
in Germany.
