Fazit

Die IgG4-SC wird als biliäre Manifestation einer IgG4-SE verstanden. IgG4-assoziierte Erkrankungen neigen zur Ausbildung tumorartiger Läsionen. So wird die IgG4-assoziierte AIP schon seit geraumer Zeit als Differenzialdiagnose des Pankreaskarzinoms betrachtet. Die IgG4-SC findet zunehmend Beachtung als Differenzialdiagnose des CCC [15]. Die korrekte Diagnose kann insbesondere bei isolierter IgG-SC sehr schwierig sein, zumal normale Serum-IgG4-Werte die Diagnose keinesfalls ausschließen. Es bleibt zu hoffen, dass in naher Zukunft bessere internationale diagnostische Leitlinien für die IgG4-SC zur Verfügung stehen werden, sodass sich unnötige chirurgische Eingriffe bei IgG-SC auf der einen und eine verzögerte OP bei CCC auf der anderen Seite vermeiden lassen.
